基于薈萃分析的HDL與肺癌相關(guān)性的評(píng)價(jià)

【摘要】:目的:系統(tǒng)評(píng)價(jià)高密度脂蛋白(HDL)與肺癌的相關(guān)性。方法:計(jì)算機(jī)檢索PubMed、ISI Web of Science、EBSCO、The Cochrane Library(2015年8期)、維普期刊資源整合服務(wù)平臺(tái)(VIP)和中國(guó)知網(wǎng)(CNKI)數(shù)據(jù)庫(kù),搜集高密度脂蛋白膽固醇(HDL-C)與肺癌相關(guān)性的研究,檢索時(shí)間均限定為建庫(kù)至2015年9月23日。由兩位研究員獨(dú)立篩選文獻(xiàn)、提取資料和評(píng)價(jià)納入研究的偏倚風(fēng)險(xiǎn)后,采用Stata 12.0軟件進(jìn)行Meta分析。結(jié)果:共納入15個(gè)研究,包括1954例患者和15545例對(duì)照。Meta分析結(jié)果顯示,HDL-C水平與肺癌發(fā)病相關(guān)[SMD=-0.66,95%CI(-0.94,-0.39),P=0.000]。亞組分析結(jié)果顯示,不同臨床分級(jí)[SMDI~II=-0.65,95%CI(-1.07,-0.23),P=0.002;SMDⅢ~Ⅳ=-0.61、95%CI(-0.73,-0.50),P=0.000]、不同病理類(lèi)型(除非小細(xì)胞肺癌)[SMD腺癌=-0.76、95%CI(-1.13,-0.38),P=0.000;SMD鱗癌=-1.51、95%CI(-2.47,-0.56),P=0.01;SMD小細(xì)胞肺癌=-1.188、95%CI(-1.42,-0.95),P=0.000]、不同研究質(zhì)量評(píng)分[SMD≥6分=-0.60、95%CI(-0.89,-0.29),P=0.000;SMD6分=-0.75、95%CI(-1.37,-0.14),P=0.017]、不同研究例數(shù)[SMD≥100例=-0.48、95%CI(-0.80,-0.15),P=0.004;SMD100例=-0.78、95%CI(-1.30,-0.26),P=0.003]、吸煙[SMD=-1.47、95%CI(-2.51,-0.43),P=0.006]以及亞洲地區(qū)[SMD=-0.89、95%CI(-1.17,-0.62),P=0.000]肺癌的發(fā)病均與HDL-C水平相關(guān)。另外,漏斗圖基本對(duì)稱(chēng),Begg's檢驗(yàn)和Egger's檢驗(yàn)顯示P值均大于0.05,提示無(wú)發(fā)表偏倚。結(jié)論:HDL水平與肺癌發(fā)病相關(guān),低HDL-C水平會(huì)增大肺癌的發(fā)生風(fēng)險(xiǎn)。鑒于納入研究的局限性,上述結(jié)論尚需進(jìn)一步開(kāi)展更多大樣本,且調(diào)整了吸煙狀態(tài)的前瞻性隊(duì)列研究加以驗(yàn)證。
[Abstract]:Objective: to evaluate the correlation between high density lipoprotein (HDL) and lung cancer. Methods: a computer-based search was made for the database of the Cochrane Library (issue 8, 2015), the VIP and CNKI databases, and the correlation between high density lipoprotein cholesterol (HDL-C) and lung cancer. The search time is limited to the construction of the database until September 23, 2015. After two researchers independently sifted the literature, extracted the data and evaluated the bias risk in the study, the Meta-analysis was carried out by Stata 12.0 software. Results: a total of 15 studies were conducted, including 1954 patients and 15545 controls. The results of Meta-analysis showed that HDL-C level was correlated with the incidence of lung cancer [SMD-0.66-95CI (-0.94U -0.39) P 0.000].
