癌癥進(jìn)化發(fā)育學(xué):基于炎-癌轉(zhuǎn)化研究的新學(xué)說(shuō)

【摘要】:慢性炎癥惡性轉(zhuǎn)化是絕大多數(shù)癌癥發(fā)生發(fā)展的共有過(guò)程。根據(jù)基因組學(xué)、體細(xì)胞變異/表觀遺傳影響的信號(hào)通路和流行病學(xué)等研究形成的證據(jù)鏈,筆者提出了癌癥進(jìn)化發(fā)育(cancer evolution-development,Cancer Evo-Dev)的新理論框架:先天免疫遺傳與后天環(huán)境暴露的交互作用引發(fā)并維持了慢性非可控性炎癥;炎癥分子持續(xù)反式激活胞苷脫氨酶,該類酶被激活后發(fā)揮炎-癌轉(zhuǎn)化橋梁的作用,誘導(dǎo)產(chǎn)生大量體細(xì)胞突變;絕大多數(shù)變異細(xì)胞被生存競(jìng)爭(zhēng)淘汰,少數(shù)則通過(guò)體細(xì)胞變異和炎癥相關(guān)表觀遺傳修飾改變了信號(hào)轉(zhuǎn)導(dǎo)模式,經(jīng)去分化過(guò)程而獲得干性特性,通過(guò)并適應(yīng)了炎癥微環(huán)境的選擇,發(fā)展成癌癥起始細(xì)胞;這一過(guò)程遵循"變異-選擇-適應(yīng)"的進(jìn)化規(guī)律。癌癥進(jìn)化發(fā)育學(xué)的提出不僅有利于闡明炎-癌轉(zhuǎn)化的一般規(guī)律,而且對(duì)癌癥的特異性預(yù)防、預(yù)測(cè)和靶向治療有重要指導(dǎo)作用。
[Abstract]:Malignant transformation of chronic inflammation is a common process in the occurrence and development of most cancers. Evidence chains based on studies of genomics, somatic variation / epigenetic effects of signal pathways and epidemiology, The authors proposed a new theoretical framework for cancer evolution-development cancer Evo-Dev: the interaction between innate immune inheritance and postnatal environmental exposure triggered and maintained chronic uncontrollable inflammation, and the persistent transactivation of cytidine deaminase by inflammatory molecules. When activated, these enzymes act as a bridge between inflammation and cancer transformation, induce a large number of somatic mutations, and most of the mutant cells are eliminated by survival competition. A few of them changed the signal transduction mode by somatic variation and epigenetic modification associated with inflammation and acquired dry properties through the process of dedifferentiation, and developed into cancer initiation cells by adapting to the choice of inflammatory microenvironment. This process follows the evolutionary law of variation-selection-adaptation. The development of cancer evolution is not only helpful to clarify the general laws of inflammation and cancer transformation, but also plays an important role in guiding the specific prevention, prediction and targeted therapy of cancer.
