【摘要】：目的構(gòu)建甲狀腺乳頭狀癌(PTC)中央?yún)^(qū)淋巴結(jié)轉(zhuǎn)移(CLNM)風(fēng)險(xiǎn)評(píng)估量表并進(jìn)行危險(xiǎn)分層。方法回顧性分析北京世紀(jì)壇醫(yī)院2010年1月至2015年6月術(shù)后病理證實(shí)為PTC的502例患者的資料,明確CLNM的獨(dú)立危險(xiǎn)因素,建立風(fēng)險(xiǎn)評(píng)估量表并進(jìn)行危險(xiǎn)分層,評(píng)估量表的預(yù)測(cè)價(jià)值,之后收集2015年7月至2016年6月術(shù)后病理證實(shí)為PTC的100例患者的資料對(duì)量表進(jìn)行驗(yàn)證。結(jié)果選取5分作為CLNM的最佳診斷界值,其靈敏度和特異性分別為73.8%和70.2%,≤4.5分CLNM風(fēng)險(xiǎn)為低危,5.0~7.0分為中危,≥7.5分為高危。使用其余100例病例對(duì)量表進(jìn)行驗(yàn)證,靈敏度為79.5%,特異性為78.7%,陽性預(yù)測(cè)值70.5%,陰性預(yù)測(cè)值85.7%。結(jié)論 CLNM風(fēng)險(xiǎn)評(píng)估量表對(duì)評(píng)估CLNM風(fēng)險(xiǎn)提供了一個(gè)簡便、直觀的量化指標(biāo),更有利于制定個(gè)體化治療方案、減少術(shù)后并發(fā)癥的發(fā)生概率。
[Abstract]:Objective to construct a risk assessment scale for central lymph node metastasis (CLNM) in papillary thyroid carcinoma (PTC). Methods the data of 502 patients with PTC confirmed by pathology from January 2010 to June 2015 in Beijing Shijitan Hospital were retrospectively analyzed. The independent risk factors of CLNM were determined, and the risk assessment scale was established and stratified to assess the predictive value of the scale. From July 2015 to June 2016, 100 patients with postoperative pathologically confirmed PTC were collected to verify the scale.
