【摘要】：目的:分析c N0甲狀腺乳頭狀癌(papillary thyroid carcinoma,PTC)對側(cè)中央?yún)^(qū)淋巴結(jié)(contralateral central lymph nodes,Cont-CLNs)轉(zhuǎn)移相關(guān)因素,探討Cont-CLNs清掃適應(yīng)證。方法:回顧性分析2013年6月至2015年12月就診于重慶醫(yī)科大學(xué)附屬第一醫(yī)院的單側(cè)PTC患者149例,均已接受甲狀腺全切以及預(yù)防性Cont-CLNs清掃。分析性別、年齡、腫瘤直徑、包膜外侵犯、原發(fā)灶數(shù)目、是否合并甲狀腺炎、喉前淋巴結(jié)、Ipsi-CLNs與Cont-CLNs轉(zhuǎn)移的關(guān)系。結(jié)果:本組患者Ipsi-CLNs轉(zhuǎn)移率和Cont-CLNs轉(zhuǎn)移率分別為73.2%和23.5%,其中性別、年齡、腫瘤直徑、原發(fā)灶數(shù)目、是否合并甲狀腺炎與Cont-CLNs轉(zhuǎn)移均不相關(guān)(P=0.792、0.097、0.531、0.578、0.269、1.000),包膜外侵犯(P=0.017)、喉前淋巴結(jié)轉(zhuǎn)移(P=0.006)和Ipsi-CLNs轉(zhuǎn)移(P0.001)與Cont-CLNs轉(zhuǎn)移相關(guān)。但多因素分析后發(fā)現(xiàn)Ipsi-CLNs轉(zhuǎn)移數(shù)目≥3枚是Cont-CLNs轉(zhuǎn)移的獨立危險因素(P=0.010)。結(jié)論:包膜外侵犯、喉前淋巴結(jié)和Ipsi-CLNs是Cont-CLNs的影響因素;當(dāng)Ipsi-CLNs轉(zhuǎn)移數(shù)目≥3枚,且合并喉前淋巴結(jié)或包膜外侵犯時,可考慮行ContCLNs清掃。
[Abstract]:Objective: to analyze the factors related to the metastasis of (papillary thyroid carcinomatous carcinoma (papillary thyroid) in the contralateral central lymph node of c N0 thyroid papillary carcinoma, and to explore the indications for the dissection of Cont-CLNs. Methods: from June 2013 to December 2015, 149 patients with unilateral PTC who were admitted to the first affiliated Hospital of Chongqing Medical University were retrospectively analyzed. All patients underwent total thyroidectomy and prophylaxis Cont-CLNs dissection. To analyze the relationship between sex, age, tumor diameter, extracapsular invasion, number of primary lesions, thyroiditis, Ipsi-CLNs in prelaryngeal lymph nodes and Cont-CLNs metastasis. Results: the metastatic rates of Ipsi-CLNs and Cont-CLNs were 73.2% and 23.5in this group respectively. There was no correlation between concomitant thyroiditis and Cont-CLNs metastasis (P0. 792 0. 097 0. 571), extracapsular invasion (P0. 017), prelaryngeal lymph node metastasis (P0. 006) and Ipsi-CLNs metastasis (P0. 001) associated with Cont-CLNs metastasis.
