【摘要】：胰腺神經(jīng)內(nèi)分泌腫瘤是一種常見的胰腺惡性腫瘤,異質(zhì)性極強(qiáng),診治手段多樣,因此,統(tǒng)一、實(shí)用的分期系統(tǒng)將有助于臨床診治及研究。目前,胰腺神經(jīng)內(nèi)分泌腫瘤包括歐洲神經(jīng)內(nèi)分泌腫瘤協(xié)會(huì)(European Neuroendocrine Tumor Society System,ENETS)和美國癌癥聯(lián)合委員會(huì)(American Joint Committee on Cancer,AJCC)兩種分期,然而兩種分期系統(tǒng)對(duì)臨床指導(dǎo)價(jià)值有限,且兩種分期系統(tǒng)的并存造成了分期系統(tǒng)混亂的局面。復(fù)旦大學(xué)附屬腫瘤醫(yī)院胰腺肝膽外科通過結(jié)合ENETS中TNM的定義及AJCC中分期的定義,建立了改良的分期系統(tǒng),該分期能更好地區(qū)分出不同預(yù)后的人群,更適合于指導(dǎo)臨床應(yīng)用,有利于建立統(tǒng)一的臨床分期標(biāo)準(zhǔn)。研究結(jié)果發(fā)表在《臨床腫瘤學(xué)雜志》(Journal of Clinical Oncology,JCO)上。研究得到了國際同行的高度重視與認(rèn)可,被評(píng)為"2017 Best of JCO:Gastrointestinal edition"。JCO編輯部進(jìn)行了專篇述評(píng)。該研究就改良分期系統(tǒng)的臨床應(yīng)用進(jìn)行解讀。
[Abstract]:Pancreatic neuroendocrine tumor is a common pancreatic malignant tumor with strong heterogeneity and various methods of diagnosis and treatment. Therefore, a unified and practical staging system will be helpful for clinical diagnosis, treatment and research. At present, pancreatic neuroendocrine tumors include two stages, the European Neuroendocrine tumor Society system (ENETs) and the American Joint Committee on Cancer (AJCC). However, the two staging systems are of limited clinical value. And the coexistence of the two staging systems caused chaos in the staging system. By combining the definition of TNM in ENETs and the definition of staging in AJCC, an improved staging system was established in the Pancreatic Hepatobiliary surgery of the affiliated Cancer Hospital of Fudan University. It is helpful to establish a unified clinical staging standard. The results were published in the Journal of Clinical Oncology (JCO). The research has been highly valued and recognized by international peers, and has been appraised as "2017 Best of JCO: Gastrointestinal edition" .JCO editorial Department has made a special review. This study interprets the clinical application of the modified staging system.
