【摘要】：目的:運(yùn)用流式細(xì)胞術(shù)檢測(cè)胃癌患者外周血T細(xì)胞亞群及CD4+T細(xì)胞、CD8+T細(xì)胞表面程序性死亡受體-1(programmed death1,PD-1)的表達(dá)情況及其與胃癌患者臨床病理特征的關(guān)系,探究其臨床意義。方法:收集山西醫(yī)科大學(xué)附屬腫瘤醫(yī)院于2015年6月至2016年6月入院且初診胃癌患者169例、健康對(duì)照組68例,采用流式細(xì)胞術(shù)檢測(cè)其外周血T淋巴細(xì)胞亞群和CD4+T、CD8+T淋巴細(xì)胞表面PD-1的表達(dá)水平,并分析手術(shù)患者PD-1表達(dá)與其TNM分期、浸潤(rùn)層面、淋巴結(jié)轉(zhuǎn)移、脈管癌栓、神經(jīng)受侵及腫瘤體積的關(guān)系。結(jié)果:1.胃癌患者外周血T淋巴細(xì)胞CD3、CD4表達(dá)水平顯著低于健康體檢組(P0.001),CD8的表達(dá)則高于健康體檢組(P0.001);2.胃癌患者外周血CD4+T、CD8+T淋巴細(xì)胞表面PD-1的表達(dá)顯著高于健康對(duì)照組(P0.001);3.CD4+T細(xì)胞表面PD-1表達(dá)與TNM分期(P0.001)、淋巴結(jié)轉(zhuǎn)移(P=0.007)、腫瘤體積(P=0.039)表達(dá)相關(guān),與浸潤(rùn)深度(P=0.402)、脈管癌栓(P=0.609)和神經(jīng)受侵(P=0.253)不相關(guān);CD8+T細(xì)胞表面PD-1表達(dá)與TNM分期(P0.001)、淋巴結(jié)轉(zhuǎn)移(P=0.004)表達(dá)相關(guān),與浸潤(rùn)層面(P=0.94)、脈管癌栓(P=0.712)、神經(jīng)受侵(P=0.671)及腫瘤體積(P=0.165)表達(dá)不相關(guān)。結(jié)論:胃癌患者外周血CD3+T淋巴細(xì)胞、CD4+T淋巴細(xì)胞表達(dá)降低,CD8+T淋巴細(xì)胞升高,并且CD4+T PD-1+、CD8+T PD-1+細(xì)胞表達(dá)顯著增高,并與胃癌患者臨床病理特征有相關(guān)性,PD-1的表達(dá)水平與胃癌的免疫逃逸過(guò)程有關(guān),并且在胃癌的發(fā)生發(fā)展中起至關(guān)重要的作用。
[Abstract]:Objective: to detect the expression of peripheral blood T cell subsets and CD4 T cell / CD8 T cell surface programmed death receptor (PCD 1) in patients with gastric cancer by flow cytometry (FCM) and its relationship with the clinicopathological features of gastric cancer patients, and to explore its clinical significance. Methods: 169 patients with gastric cancer and 68 healthy controls were collected from June 2015 to June 2016 in affiliated Cancer Hospital of Shanxi Medical University. The expression of PD-1 on peripheral blood T lymphocyte subsets and CD4 / CD8 T lymphocytes was detected by flow cytometry, and the expression of PD-1 was analyzed with TNM staging, invasive layer, lymph node metastasis, vascular tumor thrombus. The relationship between nerve invasion and tumor volume. The result is 1: 1. The expression of CD3T CD4 in peripheral blood of patients with gastric cancer was significantly lower than that in healthy controls (P 0.001) and CD8 was higher than that in healthy controls (P 0.001). The expression of PD-1 on CD4 / CD8 T lymphocytes in peripheral blood of patients with gastric cancer was significantly higher than that on normal controls. 3. The expression of PD-1 on CD4 T cells was correlated with the expression of P0.001, P0. 007 and P0. 039 in TNM staging, lymph node metastasis and tumor volume. There was no correlation between the expression of PD-1 on the surface of CD8 T cells and the expression of P0.001 in TNM staging, P0.001 in lymph node metastasis (P0.004), P0. 94 in invasive layer, P0. 712 in vascular tumor thrombus, P0. 671 in nerve invasion and P0. 165) in tumor volume. The expression of PD-1 on the surface of CD8 T cells was not correlated with the expression of P0. 001, P0. 004), P0. 712, P0. 671) and P0. 165). Conclusion: the expression of CD 3 T lymphocytes in peripheral blood of patients with gastric cancer is lower than that of CD 8 T lymphocytes, and the expression of CD 4 T PD-1 T CD 8 T PD-1 cells is significantly higher than that of CD 4 T PD-1 T cells. The expression level of PD-1 is related to the immune escape process of gastric cancer and plays an important role in the occurrence and development of gastric cancer.
