DDX3、uPAR在結(jié)直腸癌及其肝轉(zhuǎn)移中的表達(dá)及相關(guān)性分析

【摘要】:目的:通過(guò)檢測(cè)CRC及其肝轉(zhuǎn)移中DEAD-box多肽3(DDX3)及尿激酶型纖溶酶激活劑受體(uPAR)表達(dá)情況,探究DDX3和uPAR在CRC及其肝臟轉(zhuǎn)移中表達(dá)及其與CRC患者臨床病理特征(性別、年齡、腫瘤部位、分化類型、CEA、腫瘤大小、浸潤(rùn)深度、TNM分期、淋巴結(jié)轉(zhuǎn)移、肝轉(zhuǎn)移、)之間的關(guān)系。為進(jìn)一步研究DDX3、uPAR在CRC發(fā)生、發(fā)展、遠(yuǎn)處轉(zhuǎn)移尤其是肝轉(zhuǎn)移中的作用提供有用線索。方法:收集川北醫(yī)學(xué)院附屬醫(yī)院2013年01月至2015年06月CRC患者術(shù)后標(biāo)本60例,含肝轉(zhuǎn)移標(biāo)本14例;CRC癌旁正常組織標(biāo)本60例,CRC肝轉(zhuǎn)移癌旁正常組織標(biāo)本14例。采用免疫組化方法檢測(cè)標(biāo)本中DDX3、uPAR表達(dá),并進(jìn)一步分析DDX3、uPAR在CRC及其肝轉(zhuǎn)移病理組織中的表達(dá)及其與CRC患者的臨床病理特征的相互關(guān)系。所有數(shù)據(jù)均采用IBM SPSS Statistics 21.0軟件分析。結(jié)果:DDX3在CRC標(biāo)本中陽(yáng)性表達(dá)率63.33%,在CRC癌旁正常組織標(biāo)本中陽(yáng)性表達(dá)率20.00%,兩者差異有統(tǒng)計(jì)學(xué)意義(X~2=21.429,P0.05);在CRC肝轉(zhuǎn)移標(biāo)本中陽(yáng)性表達(dá)率92.86%,在CRC肝轉(zhuǎn)移癌旁正常組織標(biāo)本中陽(yáng)性表達(dá)率14.29%,兩者差異有統(tǒng)計(jì)學(xué)意義(X~2=14.359,P0.05)。uPAR在CRC標(biāo)本中陽(yáng)性表達(dá)率68.33%,在CRC癌旁正常組織標(biāo)本中陽(yáng)性表達(dá)率23.33%,兩者差異有統(tǒng)計(jì)學(xué)意義(X~2=22.691,P0.05);在CRC肝轉(zhuǎn)移標(biāo)本中陽(yáng)性表達(dá)率100.00%,在CRC肝轉(zhuǎn)移癌旁正常組織標(biāo)本中陽(yáng)性表達(dá)率7.14%,兩者差異有統(tǒng)計(jì)學(xué)意義(X~2=20.677,P0.05)。DDX3的表達(dá)在CRC患者的性別(X~2=0.027,P0.05)、年齡(X~2=0.000,P0.05)、腫瘤部位(X~2=0.058,P0.05)組間差異無(wú)統(tǒng)計(jì)學(xué)意義;而在分化類型(X~2=5.021,P0.05)、CEA量(X~2=3.305,P0.05)、腫瘤大小(X~2=3.326,P0.05)、浸潤(rùn)深度(X~2=9.569,P0.05)、TNM分期(X~2=14.712,P0.05)、淋巴結(jié)轉(zhuǎn)移(X~2=10.811,P0.05)、肝轉(zhuǎn)移(X~2=5.893,P0.05)組間差異有統(tǒng)計(jì)學(xué)意義。uPAR的表達(dá)在CRC患者的性別(X~2=0.000,P0.05)、年齡(X~2=0.000,P0.05)、腫瘤部位(X~2=0.094,P0.05)組間差異無(wú)統(tǒng)計(jì)學(xué)意義;而在分化類型(X~2=3.371,P0.05)、CEA(X~2=6.133,P0.05)腫瘤大小(X~2=6.318,P0.05)、浸潤(rùn)深度(X~2=9.569,P0.05)、TNM分期(X~2=15.561,P0.05)、淋巴結(jié)轉(zhuǎn)移(X~2=8.594,P0.05)、肝轉(zhuǎn)移(X~2=6.661,P0.05)組間差異有統(tǒng)計(jì)學(xué)意義。DDX3和uPAR均在CRC標(biāo)本中表達(dá),60例CRC標(biāo)本中共同表達(dá)33例,陽(yáng)性共表達(dá)率55.00%,采用Spearman相關(guān)分析得:(r=-0.523,-0.8r≤-0.5;P0.05),兩者呈負(fù)相關(guān),有統(tǒng)計(jì)學(xué)意義。結(jié)論:DDX3、uPAR與CRC的發(fā)生、發(fā)展、和臨床預(yù)后密切相關(guān),二者還可能共同參與了CRC的局部侵襲和遠(yuǎn)處轉(zhuǎn)移過(guò)程,并在CRC的發(fā)生、發(fā)展和遠(yuǎn)處轉(zhuǎn)移過(guò)程中相互協(xié)同、共同促進(jìn),但其與CRC局部侵潤(rùn)和分化過(guò)程,遠(yuǎn)處轉(zhuǎn)移尤其是異時(shí)性肝臟轉(zhuǎn)移的關(guān)系還有待于進(jìn)一步實(shí)驗(yàn)研究去證實(shí)。這些特征有助于DDX3、uPAR成為臨床診斷和治療CRC及其肝轉(zhuǎn)移患者新的檢測(cè)因子和治療靶點(diǎn),而同時(shí)研究DDX3和uPAR將更加有利于CRC及其肝轉(zhuǎn)移患者的臨床檢測(cè)診斷、分子靶向治療和疾病預(yù)后判斷。
[Abstract]:Objective: to investigate the expression of DDX3 and uPAR in CRC and its liver metastasis and its clinicopathological features (sex, age, tumor location) by detecting the expression of DEAD-box polypeptide 3ddx3 and urokinase type plasminogen activator receptor (UPAR) in CRC and its liver metastasis. The relationship between tumor size, TNM stage, lymph node metastasis and liver metastasis. It provides useful clues for further study on the role of DDX3 upar in the pathogenesis, development and distant metastasis of CRC, especially in liver metastasis. Methods: from January 2013 to June 2015, 60 patients with CRC and 14 patients with liver metastasis were collected from affiliated Hospital of North Sichuan Medical College. The expression of DDX3 upar was detected by immunohistochemical method. The expression of DDX3 upar in CRC and its liver metastases and their relationship with the clinicopathological features of CRC patients were analyzed. All the data were analyzed by IBM SPSS Statistics 21. 0 software. Results the positive expression rate of CRC was 63.33 in CRC and 20.00 in normal tissues adjacent to CRC. The difference between them was statistically significant (P 0.05), and 92.86 in CRC liver metastases, 92.86 in normal tissues adjacent to CRC liver metastasis. The positive expression rate was 14.29. The difference between the two groups was statistically significant. The positive expression rate of upar was 68.33 in CRC and 23.3333 in normal tissues adjacent to CRC. The difference was statistically significant between the two groups. The positive expression rate of upar in CRC liver metastases was 100.005. The positive expression rate was 7.14 in normal tissues adjacent to CRC liver metastases.
