EMT相關(guān)指標(biāo)TGF-β1、Slug及E-cadherin在膽管癌中的表達(dá)及意義

【摘要】:目的:探討TGF-β1、Slug及E-cadherin在膽管癌中的表達(dá)及臨床相關(guān)性,分析其在膽管癌發(fā)生及發(fā)展中的作用,為膽管癌的臨床治療提供實(shí)驗(yàn)依據(jù)。方法:選取2010-2016年河北醫(yī)科大學(xué)第二醫(yī)院70例有完整病理資料和臨床資料的膽管癌組織蠟塊標(biāo)本,選取20例病理均證實(shí)為陰性的膽管癌癌旁組織蠟塊標(biāo)本作為陰性對照。采用免疫組化(S-P)法檢測上述組織蠟塊中TGF-β1、Slug及E-cadherin蛋白的表達(dá)情況,并分析其與患者的年齡、腫瘤分化程度、腫瘤大小、有無淋巴結(jié)轉(zhuǎn)移、有無肝動(dòng)脈或門靜脈受侵等臨床因素的關(guān)系。用SPSS21.0統(tǒng)計(jì)軟件分析結(jié)果。結(jié)果:1 TGF-β1、Slug及E-cadherin在膽管癌組織中陽性表達(dá)率分別為67.1%、62.9%、41.4%,在癌旁正常膽管組織中的陽性表達(dá)率分別為25.0%、30.0%、80.0%,三種蛋白在膽管癌組織中和癌旁正常膽管組織中的表達(dá)差異均有統(tǒng)計(jì)學(xué)意義。2在膽管癌組織中,TGF-β1與Slug的陽性表達(dá)率在淋巴結(jié)轉(zhuǎn)移組高于無淋巴結(jié)轉(zhuǎn)移組(P0.05),在肝動(dòng)脈或門靜脈受侵組高于無受侵組(P0.05)。與病人年齡、腫瘤大小、腫瘤分化程度無相關(guān)性(P0.05)。E-cadherin的陽性表達(dá)率在淋巴結(jié)轉(zhuǎn)移組低于無淋巴結(jié)轉(zhuǎn)移組(P0.05),在肝動(dòng)脈或門靜脈受侵組低于無受侵組(P0.05)。E-cadherin的陽性表達(dá)與病人年齡、腫瘤大小、腫瘤分化程度無相關(guān)性(P0.05)。3在膽管癌組織中,TGF-β1與Slug的表達(dá)呈正相關(guān)(r=0.658,P0.05);TGF-β1與E-cadherin的表達(dá)呈負(fù)相關(guān)(r=-0.527,P0.05);Slug與E-cadherin的表達(dá)呈負(fù)相關(guān)(r=-0.526,P0.05)。結(jié)論:1與膽管癌旁正常腺體相比,TGF-β1、Slug在膽管癌中的表達(dá)明顯升高,且其表達(dá)水平與膽管癌的淋巴結(jié)轉(zhuǎn)移、肝動(dòng)脈或門靜脈受侵有關(guān)。提示二者在膽管癌發(fā)生、浸潤、轉(zhuǎn)移過程中起重要作用,可作為判斷膽管癌臨床進(jìn)展及預(yù)后的重要指標(biāo)。2與膽管癌旁正常腺體相比,E-cadherin在膽管癌中的表達(dá)明顯降低,且其表達(dá)水平與膽管癌的淋巴結(jié)轉(zhuǎn)移、肝動(dòng)脈或門靜脈受侵有關(guān)。提示其參與膽管癌發(fā)生、浸潤、轉(zhuǎn)移過程。3 E-cadherin表達(dá)的降低是EMT發(fā)生的標(biāo)志性事件,TGF-β1與Slug在調(diào)控EMT過程中存在協(xié)同作用。EMT是膽管癌發(fā)生發(fā)展過程中的重要現(xiàn)象,為膽管癌的浸潤轉(zhuǎn)移機(jī)制提供一種合理解釋。
[Abstract]:Objective: to investigate the expression of TGF- 尾 _ 1Slug and E-cadherin in cholangiocarcinoma and its clinical correlation, and to analyze the role of TGF- 尾 _ 1Slug and E-cadherin in the occurrence and development of cholangiocarcinoma, and to provide experimental evidence for the clinical treatment of cholangiocarcinoma. Methods: 70 paraffin specimens of cholangiocarcinoma tissues were collected from the second Hospital of Hebei Medical University from 2010 to 2016, and 20 samples of adjacent tissues with negative pathology were selected as negative control. The expression of TGF- 尾 1 Slug and E-cadherin protein in the wax mass was detected by immunohistochemical staining (S-P) method. The expression of TGF- 尾 1 Slug and E-cadherin protein was analyzed by immunohistochemical staining. The expression of TGF- 尾 1 Slug and E-cadherin protein was analyzed with age, tumor differentiation, tumor size and lymph node metastasis. Relationship between clinical factors such as hepatic artery or portal vein invasion. The results were analyzed by SPSS21.0 software. Results the positive expression rates of 1 TGF- 尾 1 Slug and E-cadherin in cholangiocarcinoma tissues were 67.1% and 62.9%, respectively. The positive expression rates in adjacent normal bile duct tissues were 25.010% and 80.0%, respectively. There were significant differences among the three proteins in cholangiocarcinoma tissues and adjacent normal bile duct tissues. The positive rates of TGF- 尾 1 and Slug in cholangiocarcinoma were higher in lymph node metastasis group than in non-lymph node metastasis group, and higher in hepatic artery or portal vein invasion group than in non-invasive group. The positive expression rate of P0.05U. E-cadherin in lymph node metastasis group was lower than that in non-lymph node metastasis group, and the positive expression rate in hepatic artery or portal vein group was lower than that in non-invading group, and the positive rate of E-cadherin expression was lower in hepatic artery or portal vein group than that in non-invasive group. There was a positive correlation between the expression of TGF- 尾 1 and Slug in cholangiocarcinoma tissues. There was a negative correlation between the expression of TGF- 尾 1 and E-cadherin. There was a negative correlation between the expression of TGF- 尾 1 and E-cadherin. Conclusion the expression of TGF- 尾 1 Slug in cholangiocarcinoma is significantly higher than that in adjacent normal glands. The expression level of TGF- 尾 1 Slug is related to lymph node metastasis, hepatic artery or portal vein invasion of cholangiocarcinoma. These results suggest that the expression of E-cadherin in cholangiocarcinoma is significantly lower than that in the normal gland adjacent to cholangiocarcinoma, and it may play an important role in the development, invasion and metastasis of cholangiocarcinoma. It can be used as an important index to judge the clinical progress and prognosis of cholangiocarcinoma. The expression level was related to lymph node metastasis, hepatic artery or portal vein invasion of cholangiocarcinoma. It is suggested that the decrease of E-cadherin expression in the process of carcinogenesis, invasion and metastasis of cholangiocarcinoma is a landmark event of EMT. TGF- 尾 1 and Slug have synergistic effect on the regulation of EMT. EMT is an important phenomenon in the process of occurrence and development of cholangiocarcinoma. It provides a reasonable explanation for the mechanism of invasion and metastasis of cholangiocarcinoma.
