【摘要】：目的:本文通過meta分析,系統(tǒng)性地分析了近十年來腹腔鏡下直腸癌切除術(shù)后吻合口瘺的危險因素,更好地為臨床上的治療提供決策。方法:根據(jù)納入與排除標(biāo)準(zhǔn),系統(tǒng)檢索英文數(shù)據(jù)庫(Pub Med、Embase、The Cochrane Library)和中文數(shù)據(jù)庫(中國知網(wǎng)、萬方數(shù)據(jù)庫)中關(guān)于分析腹腔鏡直腸癌術(shù)后吻合口瘺危險因素隊列研究的相關(guān)文獻,檢索時間設(shè)置為2007年1月至2017年1月,經(jīng)過瀏覽題目、摘要及全文后,篩出符合納入標(biāo)準(zhǔn)的高質(zhì)量文獻,摘錄其中數(shù)據(jù)。運用Review Manager 5.3進行Meta分析。結(jié)果:最終19篇文獻入選本次Meta分析,總樣本量8422例,總體吻合口瘺發(fā)生率5.88%(495/8422)。Meta分析結(jié)果顯示:性別[合并OR值2.41,95%CI(1.93-3.00),P0.00001];術(shù)前白蛋白[合并OR值0.59,95%CI(0.38-0.90),P=0.01];糖尿病[合并OR值1.48,95%CI(1.06-2.07),P=0.02];術(shù)前BMI[合并OR值1.64,95%CI(1.01-2.65),P=0.005];術(shù)前新輔助放化療[合并OR值1.73,95%CI(1.27-2.36),P=0.0005];腫瘤距肛緣距離[合并OR值0.60,95%CI(0.46-0.77),P0.0001];腫瘤大小[合并OR值2.10,95%CI(1.35-3.27),P=0.001]。結(jié)論:男性、術(shù)前白蛋白3.5mg/d L、術(shù)前合并糖尿病、BMI≥25kg/m2、術(shù)前行新輔助放化療、腫瘤距肛緣距離≤7厘米和腫瘤直徑≥5厘米是腹腔鏡下直腸癌術(shù)后吻合口瘺的重要危險因素
[Abstract]:Objective: to systematically analyze the risk factors of anastomotic leakage after laparoscopic resection of rectal cancer in recent ten years by meta analysis. Methods: according to the criteria of inclusion and exclusion, we systematically searched the English database Pub Medbase, the Cochrane Library, and the Chinese database, which were related to the cohort study on the risk factors of anastomotic fistula after laparoscopic rectal cancer surgery. The retrieval time was set from January 2007 to January 2017. After browsing the title, summary and full text, the high quality documents which met the inclusion criteria were selected and the data were extracted. Review Manager 5.3 was used for Meta analysis.
