肺癌患者血清NGAL的水平及意義

【摘要】:目的探討肺癌患者血清中性粒細胞明膠酶相關(guān)脂質(zhì)運載蛋白(neutrophil gelatinase-associated lipocalin,NGAL)的水平變化及意義,以期有助于肺癌的早期診斷及治療。方法選取2014年10月至2016年10月在鄭州大學第五附屬醫(yī)院經(jīng)病理確診且是初診未行放、化療治療的肺癌患者為研究對象,共入選肺癌患者112例(肺癌組);并選取同期在鄭州大學第五附屬醫(yī)院體檢中心體檢的112例健康人(對照組)。記錄所有受試者的一般資料及肺癌組患者的組織病理及免疫組化結(jié)果,并在空腹12h后采集所有受試者的靜脈血樣,應(yīng)用酶聯(lián)免疫吸附法測定肺癌組與健康對照組的血清NGAL水平。采用SPSS 19.0統(tǒng)計學軟件進行相關(guān)分析,分別采用t檢驗、χ2檢驗、單因素方差分析等比較組間差異,然后采用多因素Logistic回歸分析肺癌發(fā)生、發(fā)展及遠處轉(zhuǎn)移的危險因素。結(jié)果肺癌組血清NGAL水平高于對照組(P0.05);肺癌組吸煙指數(shù)(smoking index,SI)明顯高于對照組(P0.01);不同性別、年齡、吸煙史間兩組患者血清NGAL水平均無顯著性差異;肺癌不同部位、大小、病理類型及有無區(qū)域淋巴結(jié)轉(zhuǎn)移間,血清NGAL水平的差異均無統(tǒng)計學意義;低分化肺癌患者血清NGAL水平高于高分化肺癌患者,有遠處轉(zhuǎn)移的肺癌患者血清NGAL水平顯著高于無遠處轉(zhuǎn)移者;Ⅳ期患者的血清NGAL水平均顯著高于Ⅰ期+Ⅱ期;多因素回歸分析顯示,血清NGAL(OR=2.942,P=0.021)、吸煙指數(shù)(OR=8.610,P=0.013)與肺癌的發(fā)生有關(guān),且血清NGAL(OR=5.603,P=0.035)與肺癌遠處轉(zhuǎn)移有關(guān)。結(jié)論1、NGAL在肺癌患者血清中高表達;2、NGAL與肺癌的發(fā)生有關(guān),且NGAL與肺癌的分化程度、遠處轉(zhuǎn)移、臨床分期有關(guān),可在一定程度上反映肺癌的侵襲性、評估肺癌患者的預(yù)后。
[Abstract]:Objective to investigate the changes and significance of serum neutrophil gelatinase-associated lipocalin NGALs in patients with lung cancer in order to help the early diagnosis and treatment of lung cancer. Methods from October 2014 to October 2016, lung cancer patients who had been diagnosed pathologically and were treated with chemotherapy and without radiotherapy were selected as the study subjects, who were admitted to the Fifth affiliated Hospital of Zhengzhou University from October 2014 to October 2016. 112 patients with lung cancer (lung cancer group) and 112 healthy persons (control group) who were examined in the medical examination center of the Fifth affiliated Hospital of Zhengzhou University at the same time were selected. The general data of all subjects and the histopathological and immunohistochemical results of lung cancer patients were recorded. The venous blood samples of all subjects were collected 12 hours after an empty stomach. The serum NGAL levels in lung cancer group and healthy control group were measured by enzyme linked immunosorbent assay (Elisa). Correlation analysis was carried out with SPSS 19.0 software. T test, 蠂 2 test and univariate ANOVA were used to compare the differences between groups, and then multivariate Logistic regression analysis was used to analyze the risk factors of lung cancer occurrence, development and distant metastasis. Results Serum NGAL level in lung cancer group was higher than that in control group (P 0.05), smoking index in lung cancer group was significantly higher than that in control group (P 0.01), there was no significant difference in serum NGAL level between two groups with different sex, age, smoking history, different location and size of lung cancer. There was no significant difference in serum NGAL level between the pathological types and regional lymph node metastasis, and the serum NGAL level in poorly differentiated lung cancer patients was higher than that in well-differentiated lung cancer patients.
