【摘要】：目的比較腹腔鏡下保留腎上腺與腎上腺全切除術(shù)治療醛固酮腺瘤的圍術(shù)期安全性及術(shù)后療效。方法在線系統(tǒng)地檢索Pub Med、Science Direct、Springerlink、the Cochrane Library、中國知網(wǎng)、中國生物醫(yī)學(xué)文獻(xiàn)數(shù)據(jù)庫于2016年5月之前發(fā)表的關(guān)于保留腎上腺(ASS)或腎上腺部分切除術(shù)(PA)對比腎上腺全切除術(shù)(TA)治療醛固酮腺瘤(APA)的文獻(xiàn)資料,將入選的研究文獻(xiàn)應(yīng)用Revman 5.3軟件進(jìn)行meta分析,分析的主要內(nèi)容是圍術(shù)期結(jié)果(手術(shù)時(shí)間、術(shù)中出血量、住院時(shí)間)以及術(shù)后療效(治愈率、部分緩解率、無效率)。結(jié)果最終共納入9篇臨床研究文獻(xiàn),其中3篇英文文獻(xiàn),6篇中文文獻(xiàn),共包括1036例患者,其中ASS組544例,TA組492例。結(jié)果顯示,ASS對比TA的手術(shù)時(shí)間(WMD: 2.09min,95%CI: 9.86~5.67,P=0.60)、術(shù)中出血量(WMD:1.13ml,95%CI: 8.86~11.12,P=0.82)、住院時(shí)間(WMD: 0.10d,95%CI: 0.32~0.12,P=0.36)差異無統(tǒng)計(jì)學(xué)意義,而ASS對比TA在治愈率(OR=1.07,95%CI:0.73~1.58,P=0.72)、部分緩解率(OR=0.85,95%CI:0.57~1.27,P=0.43)、無效率(OR=2.15,95%CI:0.32~14.34,P=0.43)差異亦無統(tǒng)計(jì)學(xué)意義。結(jié)論在醛固酮腺瘤的手術(shù)治療中,與腎上腺全切除術(shù)比較,保留腎上腺手術(shù)技術(shù)上安全可靠,術(shù)后療效肯定,二者具有相似的治療效果,值得進(jìn)一步在臨床上推廣。
[Abstract]:Objective to compare the perioperative safety and postoperative efficacy of laparoscopic adrenalectomy and total adrenalectomy for aldosterone adenoma. Methods Pub Medsite Science DirectLink and the Cochrane Library were systematically searched online. The Chinese Biomedical Literature Database published before May 2016 published literature on the treatment of aldosterone adenoma (APA) by preserving the adrenal gland (ASS) or partial adrenalectomy (PAA) compared with total adrenalectomy (TAA). Revman 5.3 software was used for meta analysis. The main contents of the analysis were perioperative results (operative time, intraoperative bleeding volume, hospitalization time) and postoperative efficacy (cure rate, partial remission rate, inefficiency).
