【摘要】：目的 :分析特殊部位轉移的乳腺癌的臨床病理特征及預后。方法 :回顧性分析2012—2015年68例伴特殊部位轉移的乳腺癌患者的臨床資料。特殊轉移部位的定義:除淋巴結、胸壁、乳腺、骨、肝、肺、腦、腹膜和胸膜轉移以外的少見轉移部位。結果:68例患者中,最常見的特殊轉移部位依次為腎上腺(44.1%,30/68)、卵巢(41.2%,28/68)和胰腺(11.8%,8/68),其余包括眼、子宮、脾、直腸、膀胱、腎、甲狀腺和腮腺等。75.0%(51/68)的患者伴有其他常見部位的轉移。特殊部位轉移后的中位生存時間為23.5個月(范圍:1.4~116.0個月),不伴有常見部位轉移患者的中位生存時間與伴有常見部位轉移患者相比,前者顯示出生存優(yōu)勢(分別為36.0和18.0個月,P=0.007)。結論 :乳腺癌轉移部位廣泛,除常見轉移部位以外,還可以轉移至許多少見部位;不伴有常見部位轉移的特殊部位轉移乳腺癌患者的預后較好,因此有選擇性地進行局部治療可能會改善生存;卵巢轉移大多于去勢手術時發(fā)現,臨床上在選擇卵巢去勢方式時,應考慮卵巢轉移的可能性。
[Abstract]:Objective: to analyze the clinicopathological features and prognosis of breast cancer with special site metastasis. Methods: the clinical data of 68 breast cancer patients with special site metastasis from 2012 to 2015 were analyzed retrospectively. Definition of special metastatic sites: rare metastatic sites other than lymph nodes, chest walls, mammary glands, bone, liver, lung, brain, peritoneum, and pleural metastases. Results among 68 patients, the most common special metastases were the adrenal gland 44.1 / 68, the ovary 41.2 / 68) and the pancreas 11.8 / 68. The rest included eyes, uterus, spleen, rectum, bladder, kidney, thyroid, parotid gland, etc. The median survival time of the patients with special site metastasis was 23.5 months (range: 1.4 ~ 116.0 months). The median survival time of the patients without common site metastasis showed a survival advantage (36.0 and 18.0 months, respectively) compared with that of the patients with common site metastasis (36.0 and 18.0 months, respectively). Conclusion: the metastatic sites of breast cancer are extensive and can be transferred to many rare sites in addition to the common metastatic sites. Therefore selective local treatment may improve survival and ovarian metastases are mostly found in ovariectomies and the possibility of ovarian metastasis should be taken into account when selecting ovariectomized methods in clinic.
