MicroRNA-210與微小殘留病灶的聯(lián)合檢測(cè)在兒童急性淋巴細(xì)胞白血病中的預(yù)后意義

【摘要】:目的:檢測(cè)MicroRNA-210(miR-210)在兒童急性淋巴細(xì)胞白血病(ALL)中的表達(dá)水平,探討miR-210與微小殘留病灶(MRD)聯(lián)合檢測(cè)對(duì)指導(dǎo)臨床治療及判斷預(yù)后的意義。方法:采用RQ-PCR方法檢測(cè)88例初診兒童ALL骨髓樣本中miR-210的表達(dá)水平及第33天的MRD表達(dá)水平。結(jié)果:miR-210在兒童ALL初診骨髓樣本中普遍高表達(dá),且在非復(fù)發(fā)組miR-210的表達(dá)水平顯著高于復(fù)發(fā)組(10.64±1.5 vs 3.27±0.68)(P0.05),KaplanMeier生存分析結(jié)果表明,與高表達(dá)組相比,miR-210低表達(dá)組具有較差的無(wú)復(fù)發(fā)活存率(RFS)(P=0.011)、無(wú)事件活存率(EFS)(P=0.013)及總活存率(OS)(P=0.0108)。根據(jù)miR-210及MRD水平,將88例患兒分為3組。miR-210-MRD高風(fēng)險(xiǎn)組復(fù)發(fā)率(70%)顯著高于miR-210-MRD中風(fēng)險(xiǎn)組(6.25%)及miR-210-MRD低風(fēng)險(xiǎn)組(2.1%)。Kaplan-Meier生存分析結(jié)果表明,miR-210-MRD高風(fēng)險(xiǎn)組RFS、EFS及OS均顯著低于中風(fēng)險(xiǎn)組和低風(fēng)險(xiǎn)組(P0.01)。結(jié)論:兒童ALL初診骨髓樣本中miR-210的表達(dá)水平對(duì)疾病復(fù)發(fā)、誘導(dǎo)失敗有良好的預(yù)測(cè)價(jià)值。miR-210的低表達(dá)與低LFS、EFS及OS高度相關(guān)。miR-210與第33天MRD水平的聯(lián)合檢測(cè),可以更準(zhǔn)確地識(shí)別出預(yù)后差、復(fù)發(fā)風(fēng)險(xiǎn)大的患兒。
[Abstract]:Objective: to detect the expression level of MicroRNA-210 (miR-210) in children's acute lymphoblastic leukemia (ALL) and explore the significance of combined detection of miR-210 and minimal residual focus (MRD) in guiding clinical treatment and judging prognosis. Methods: RQ-PCR method was used to detect the expression of miR-210 in 88 cases of newly diagnosed children's ALL bone marrow samples and the MRD table for the 33 day. Results: miR-210 was highly expressed in the children's ALL first diagnosed bone marrow samples, and the expression level of miR-210 in the non recurrent group was significantly higher than that in the recurrent group (10.64 + 1.5 vs 3.27 + 0.68) (P0.05). The KaplanMeier survival analysis showed that the low expression group of miR-210 had poor recurrence free survival rate (RFS) (P=0.011) and no matter compared with the high expression group. The survival rate (EFS) (P=0.013) and total survival rate (OS) (P=0.0108). According to the miR-210 and MRD levels, the recurrence rate of the 88 children was divided into 3 groups of.MiR-210-MRD high risk groups (70%), which were significantly higher than those in the miR-210-MRD risk group (6.25%) and the miR-210-MRD low risk group (2.1%).Kaplan-Meier survival analysis.
