【摘要】：目的:分析直腸惡性腫瘤手術(shù)患者的住院費(fèi)用負(fù)擔(dān)情況,探討其影響因素和分組路徑,為醫(yī)療機(jī)構(gòu)費(fèi)用管理作借鑒,為減輕患者負(fù)擔(dān)作參考。方法:回顧搜集2014年1月—2015年12月直腸腫瘤住院手術(shù)患者數(shù)據(jù),應(yīng)用樹(shù)模型來(lái)推斷住院費(fèi)用負(fù)擔(dān)的相關(guān)影響因素。結(jié)果:根治手術(shù)、超聲刀與高頻電刀的使用與住院費(fèi)用可能存在統(tǒng)計(jì)學(xué)關(guān)聯(lián),造瘺手術(shù)與否、DRGs分組是患者住院費(fèi)用及基金申報(bào)的主要判別因素。結(jié)論:醫(yī)保待遇差異影響患者負(fù)擔(dān)水平,手術(shù)術(shù)式及DRGs分組影響病種疾病負(fù)擔(dān),一方面政府應(yīng)充分發(fā)揮醫(yī)保的引導(dǎo)和杠桿作用,另一方面醫(yī)療機(jī)構(gòu)宜細(xì)化病種費(fèi)用管理、強(qiáng)化病種成本核算,共同推動(dòng)腫瘤專業(yè)的三醫(yī)聯(lián)動(dòng)。
[Abstract]:Objective: to analyze the burden of hospitalization expenses of patients with rectal malignant tumor, and to explore its influencing factors and grouping path, so as to provide reference for the management of medical institutions' expenses and to reduce the burden of patients.Methods: data of inpatients with rectal neoplasms from January 2014 to December 2015 were collected retrospectively and the related factors of hospitalization cost burden were inferred by tree model.Results: the use of radical operation, ultrasonic knife and high frequency electric knife may be statistically related to the hospitalization expenses. The main discriminant factors of hospital expenses and fund declaration were whether or not the group of DRGs was used in the operation of fistula.Conclusion: the difference of medical insurance treatment affects the burden level of patients, and the operation method and DRGs group affect the burden of diseases. On the one hand, the government should give full play to the guidance and leverage of medical insurance, on the other hand, the medical institutions should refine the management of disease cost.Strengthen the cost accounting of disease, promote cancer professional three-doctor linkage.
