【摘要】：目的:觀察結(jié)直腸癌術(shù)后使用FOLFOX6方案化療期間聯(lián)合中藥治療的臨床療效及不良反應(yīng)。方法:將60例結(jié)直腸癌術(shù)后的患者隨機(jī)分為2組,中藥組(30例)化療期間聯(lián)合中藥治療,對(duì)照組(30例)單純化療。化療方案:FOLFOX6方案為:奧沙利鉑注射液以每平方米85mg的量第一天靜脈滴注,并且于2小時(shí)內(nèi)輸入完畢,亞葉酸鈣注射液以每平方米400 mg的量第一天靜脈滴注,并且于2小時(shí)內(nèi)輸入完畢,氟尿嘧啶注射液每平方米400 mg于第一天靜脈推注,氟尿嘧啶注射液每平方米1200mg持續(xù)靜脈滴注兩天(總量每平方米2400mg輸注46-48h)于第一天開(kāi)始,每2周重復(fù)1次,采用x2檢驗(yàn)及t檢驗(yàn),對(duì)兩組間中醫(yī)證候療效、中醫(yī)癥狀積分、外周血細(xì)胞計(jì)數(shù)、消化道反應(yīng)、肝功能、神經(jīng)毒性反應(yīng)和T細(xì)胞亞群進(jìn)行統(tǒng)計(jì)學(xué)分析。結(jié)果:療程結(jié)束后中藥組與對(duì)照組中醫(yī)證候療效有效率分別為70.00%、26.67%(P0.05),對(duì)應(yīng)中醫(yī)癥狀積分分別為8.36±3.81、15.40±4.90(P0.05)。兩組患者化療后實(shí)驗(yàn)室檢查的比較:中藥組在減少肝功能損傷的效果明顯優(yōu)于對(duì)照組,其差異性有統(tǒng)計(jì)學(xué)意義(P0.05),而白細(xì)胞的變化無(wú)統(tǒng)計(jì)學(xué)意義,主要消化道反應(yīng)和神經(jīng)毒性反應(yīng)相比差異有統(tǒng)計(jì)學(xué)意義(P0.05),兩組CD4+(%)和CD8+(%)比較分別為:43.40±5.09 vs 35.09±5.28,30.20±5.63vs 34.13±5.90,中藥組CD4+(%)較對(duì)照組明顯升高,而CD8+(%)明顯減低(均P0.05)。結(jié)論:結(jié)直腸癌術(shù)后化療時(shí)配合中藥口服治療,可以減少結(jié)直腸癌患者術(shù)后的并發(fā)癥改善其生存質(zhì)量,還可以提高患者的免疫力,效果顯著,值得在今后的臨床工作中廣泛使用。
[Abstract]:Objective: to observe the clinical effect and adverse reaction of FOLFOX6 regimen combined with Chinese medicine after colorectal cancer operation.Methods: sixty patients with colorectal cancer after operation were randomly divided into two groups: the Chinese medicine group (n = 30) and the control group (n = 30).Chemotherapy regimen: FOLFOX6: oxaliplatin injection was given intravenously per square meter of 85mg on the first day, and was infused within 2 hours, and calcium folate injection was given intravenously at 400 mg per square meter on the first day.At the end of the infusion within 2 hours, fluorouracil injection (400mg per square meter) was injected intravenously on the first day, and fluorouracil injection per square meter 1200mg continued for two days (46-48 hours per square meter of 2400mg) from the first day.Every 2 weeks, the effect of TCM syndrome, TCM symptom score, peripheral blood cell count, digestive tract reaction, liver function, neurotoxic reaction and T cell subgroup were statistically analyzed by x2 test and t test.Results: after the course of treatment, the effective rates of TCM syndromes in the TCM group and the control group were 70.00,26.67 and 8.36 鹵3.81 鹵15.40 鹵4.90, respectively, and the corresponding scores of TCM symptoms were 8.36 鹵3.81 and 15.40 鹵4.90, respectively.Comparison of laboratory examination after chemotherapy in two groups: the effect of traditional Chinese medicine group on reducing liver function injury was significantly better than that of control group, the difference was statistically significant (P 0.05), but the change of white blood cell was not statistically significant.There was significant difference in the main digestive tract reaction and neurotoxic reaction between the two groups (P 0.05). The ratio of CD4 and CD8 were 43.40 鹵5.09 vs 35.09 鹵5.2830.20 鹵34.13 鹵5.90, respectively. The CD4 in the Chinese medicine group was significantly higher than that in the control group, but CD8 was significantly decreased (P 0.05).Conclusion: the postoperative chemotherapy combined with oral Chinese medicine can reduce the postoperative complications and improve the quality of life of patients with colorectal cancer, and it can also improve the immunity of patients with colorectal cancer.It is worthy to be widely used in clinical work in the future.
