【摘要】：目的總結(jié)原發(fā)性腎鱗狀細(xì)胞癌的臨床和病理特征,分析腎鱗狀細(xì)胞癌容易誤診的原因,提高對(duì)腎鱗狀細(xì)胞癌的診治水平。方法回顧性分析2例被誤診的原發(fā)性腎鱗狀細(xì)胞癌病例的臨床、病理資料,并結(jié)合文獻(xiàn)分析其常見病因、發(fā)病機(jī)制、臨床表現(xiàn)及診治。結(jié)果病例1行左腎及部分左側(cè)輸尿管切除術(shù),病理結(jié)果提示:左腎盂鱗癌,癌細(xì)胞侵犯輸尿管斷端。病例2行根治性右腎腫瘤切除術(shù),術(shù)后病理結(jié)果提示:右腎高分化的鱗狀細(xì)胞癌,右腎腎周脂肪發(fā)現(xiàn)鱗癌細(xì)胞,腎盂及輸尿管未見累及。2例術(shù)后均未行放療、化療,病例1術(shù)后3月死于癌細(xì)胞轉(zhuǎn)移,病例2術(shù)后4月死于癌細(xì)胞轉(zhuǎn)移。結(jié)論原發(fā)性腎臟鱗狀細(xì)胞癌臨床少見,患者早期多無明顯臨床癥狀,無明顯包塊的鱗狀細(xì)胞癌影像學(xué)并不敏感,輸尿管鏡及軟鏡下取組織活檢也有一定局限性,臨床上容易漏診、誤診。
[Abstract]:Objective to summarize the clinical and pathological features of primary renal squamous cell carcinoma (RSCC), to analyze the causes of misdiagnosis of RSCC, and to improve the diagnosis and treatment of renal squamous cell carcinoma (RSCC).Methods the clinical and pathological data of 2 misdiagnosed cases of primary renal squamous cell carcinoma were analyzed retrospectively. The common etiology, pathogenesis, clinical manifestation, diagnosis and treatment were analyzed in combination with literature.Results in case 1, the left kidney and part of the left ureterectomy were performed. The pathological results showed that the carcinoma of the left renal pelvis invaded the broken end of the ureter.Case 2 underwent radical resection of right renal tumor. The pathological results showed that: squamous cell carcinoma of the right kidney was well differentiated, squamous cell carcinoma was found in perirenal fat of the right kidney, no involvement of renal pelvis and ureter was observed in 2 cases, no radiotherapy and chemotherapy were performed after operation.Case 1 died of cancer metastasis 3 months after operation and case 2 died of cancer metastasis 4 months after operation.Conclusion Primary renal squamous cell carcinoma is rare in clinical practice. Most of the patients have no obvious clinical symptoms in the early stage. The imaging of squamous cell carcinoma without obvious mass is not sensitive, and biopsy under ureteroscope and soft microscope has some limitations.It is easy to miss diagnosis and misdiagnose clinically.
