【摘要】：研究背景:女性生殖系統(tǒng)惡性腫瘤中,上皮性卵巢癌發(fā)病率位居第三。由于卵巢癌缺乏明顯的早期臨床表現(xiàn)且臨床上無特異敏感的早期診斷指標(biāo),研究表明臨床上超過75%的卵巢癌患者在初診時已達(dá)晚期,而卵巢癌晚期患者往往具有易復(fù)發(fā)性和藥物耐受性,因此根據(jù)臨床統(tǒng)計結(jié)果顯示卵巢癌患者的五年生存率僅25%-50%。RNA結(jié)合蛋白是近年來研究發(fā)現(xiàn)的具有重要生物學(xué)功能的分子家族,其能通過調(diào)節(jié)基因的轉(zhuǎn)錄后表達(dá)水平影響細(xì)胞的生物學(xué)特性,參與人體許多生理病理進(jìn)程,其中腫瘤的生物學(xué)進(jìn)程是當(dāng)今社會的研究重點和科研難點。QKI編碼蛋白屬于RNA結(jié)合蛋白,其能通過與m RNA上的QKI反應(yīng)原件(QREs)結(jié)合從而影響m RNA的穩(wěn)定及表達(dá),調(diào)控RNA的轉(zhuǎn)錄和分布。研究表明,QKI-5的缺失或部分缺失能影響中樞神經(jīng)系統(tǒng)的發(fā)育,且在肺癌,結(jié)直腸癌,口腔癌,胃癌,乳腺癌等癌癥中QKI-5都存在明顯的表達(dá)下調(diào),相關(guān)體內(nèi)體外實驗結(jié)果也證實,QKI-5的表達(dá)下調(diào)與腫瘤的發(fā)生發(fā)展有重要的意義。Ezrin和SLC16A3分別作為重要的膜-細(xì)胞骨架蛋白和膜轉(zhuǎn)運蛋白也在腫瘤的生物學(xué)進(jìn)程中扮演重要的角色,研究表明SLC16A3和Ezrin在非小細(xì)胞肺癌(NSCLC)、導(dǎo)管內(nèi)乳頭狀乳腺癌、前列腺癌和骨肉瘤等惡性腫瘤中存在明顯的差異表達(dá),Ezrin能通過改變細(xì)胞結(jié)構(gòu)穩(wěn)定性、調(diào)節(jié)細(xì)胞信號傳導(dǎo)通路等途徑參與腫瘤的發(fā)生發(fā)展而SLC16A3則通過調(diào)節(jié)細(xì)胞內(nèi)的乳酸濃度影響腫瘤細(xì)胞的代謝水平最終影響其存活狀態(tài)。然而,在卵巢癌中QKI-5、SLC16A3和Ezrin的相關(guān)性研究卻很少,且缺乏相應(yīng)的機制研究。研究方法:1、采用組織芯片技術(shù)檢測QKI-5、Ezrin和SLC16A3在134例漿液性卵巢癌組織和18例正常的卵巢搔刮上皮細(xì)胞中的差異表達(dá)情況,并通過統(tǒng)計學(xué)方法分析各個基因表達(dá)趨勢與臨床病理的相關(guān)性。2、采用Q-PCR技術(shù)檢測40例新鮮卵巢癌組織樣品和27例正常卵巢上皮細(xì)胞中QKI-5、Ezrin和SLC16A3在m RNA水平的差異表達(dá)。。3、構(gòu)建QKI-5和SLC16A3的過表達(dá)及敲低質(zhì)粒,采用增殖、侵襲、遷移、周期以及凋亡實驗檢測其對A2780和SKOV3細(xì)胞功能的調(diào)節(jié)作用。通過藥物敏感實驗檢測SLC16A3對卵巢癌細(xì)胞SKOV3和A2780鉑類化療藥物耐藥性的影響。4、通過裸鼠皮下成瘤實驗和NOD/SCID鼠尾靜脈轉(zhuǎn)移實驗分別檢測QKI-5對卵巢癌細(xì)胞成瘤性以及血運轉(zhuǎn)移能力的影響。研究結(jié)果:1、在134例卵巢癌病例調(diào)查中發(fā)現(xiàn)QKI-5和Ezrin的表達(dá)明顯低于正常對照組,QKI-5和EZRIN的表達(dá)下調(diào)與卵巢癌患者的分期、大網(wǎng)膜轉(zhuǎn)移情況、生存時間有重要的相關(guān)性(p0.05)。而SLC16A3在卵巢癌中表達(dá)明顯上調(diào),且其表達(dá)上調(diào)的患者具有更強的化療藥物耐受性(p0.05)。2、Q-PCR結(jié)果顯示:與正常卵巢上皮相比,QKI-5和Ezrin在卵巢癌組織中呈下調(diào)表達(dá),而SLC16A3則明顯表達(dá)上調(diào),且差異具有統(tǒng)計學(xué)意義。3、細(xì)胞實驗結(jié)果顯示:在SKOV3和A2780細(xì)胞中QKI-5能增加細(xì)胞的增殖、遷移和侵襲能力,并能增加細(xì)胞G1期比例;動物實驗結(jié)果顯示:上調(diào)表達(dá)QKI-5能降低卵巢癌細(xì)胞系的成瘤能力并能減弱其血行轉(zhuǎn)移能力。4、SLC16A3能增強卵巢癌細(xì)胞的增殖能力,并且能加強癌細(xì)胞穿過組織間隙和血管壁的能力,上調(diào)細(xì)胞S期比例,SLC16A3能通過增強SKOV3和A2780對鉑類藥物的耐藥性,促進(jìn)腫瘤的生長和抗凋亡能力。結(jié)論:1、QKI-5、Ezrin和SLC16A3在卵巢癌中存在異常表達(dá),且具有顯著的臨床意義。2、QKI-5和SLC16A3在卵巢癌的生物學(xué)進(jìn)程中分別發(fā)揮著腫瘤抑制和促進(jìn)腫瘤發(fā)生發(fā)展的作用。3、QKI-5、SLC16A3和Ezrin有望成為卵巢癌早期診斷指標(biāo),并為卵巢癌患者預(yù)后判斷及臨床治療提供新思路。

The results show that QKI-5 loss or partial loss can affect the development of the central nervous system, and in lung cancer, colorectal cancer, oral cancer, gastric cancer, breast cancer and other cancers in QKI-5 are significantly down regulated. Correlation in vivo and in vitro experimental results also confirmed that the occurrence and development of down-regulation of QKI-5 expression and tumor have important significance of.Ezrin and SLC16A3 were used as the important membrane cytoskeleton proteins and membrane transporters play an important role in the biological processes of tumor, it is shown that the SLC16A3 and Ezrin in non-small cell lung cancer (NSCLC), catheter papillary breast cancer, the expression differences of prostate cancer and osteosarcoma malignant tumors, Ezrin can change the cell structure stability, regulation of cell signal transduction pathway and other pathways involved in tumor The occurrence and development of SLC16A3 by regulating the intracellular lactate concentration affect the tumor cell metabolism and ultimately affect its survival state. Conclusion: 1, QKI-5, Ezrin and SLC16A3 in there is abnormal expression in ovarian cancer, and has significant clinical significance of.2, QKI-5 and SLC16A3 respectively play a role in tumor suppression and.3, the occurrence and development of tumor QKI-5 in the biological process of ovarian cancer, SLC16A3 and Ezrin It is expected to be an early diagnostic index for ovarian cancer and provides a new way of thinking for prognosis and clinical treatment of ovarian cancer patients.
