SALL4和Bmi-1在宮頸癌組織中的表達及臨床意義

【摘要】:目的探討人婆羅雙樹樣基因-4蛋白(Sal-like protein 4,SALL4)和B細胞特異的莫洛尼白血病毒插入點1基因蛋白(B-cell specific moloney leukemia virus insertion site 1,Bmi-1)的表達在宮頸癌發(fā)生發(fā)展過程中的意義。方法病例研究選取:患者住院時間2014年9月到2016年9月手術并確診的宮頸癌、宮頸上皮內(nèi)病變(CIN)患者以及正常健康宮頸組織。隨機選取宮頸癌患者30例,其中鱗癌患者22例,腺癌患者8例,年齡25~69歲,中位年齡45歲。選取CIN患者60例,包括低級別CIN 30例和高級別CIN 30例,年齡22~68歲,中位年齡46歲。同時收集正常宮頸組織標本30例設置為對照組。本研究中所有的宮頸組織標本在患者術后均經(jīng)過病理科病理確診,且患者在術前均未接受放療、化療或任何激素治療的病例。三組宮頸組織中的SALL4和Bmi-1表達水平采用免疫組織化學技術(SP法)檢測。采用SPSS19.0統(tǒng)計軟件進行數(shù)據(jù)分析,數(shù)據(jù)的集中趨勢和離散趨勢(`X±S)表示,P0.05作為差別具有統(tǒng)計學意義的標準。應用卡方檢驗分析各組之間所測指標的差異。Pearson法分析SALL4和Bmi-1表達水平的相關性。結果SALL4陽性表達率:正常宮頸組織、低級別宮頸上皮內(nèi)病變、高級別宮頸上皮內(nèi)病變和宮頸癌組織中的表達分別是10.00%、36.67%、63.33%、86.67%(P0.05);采用SPSS19.0統(tǒng)計軟件進行組間差異應用卡方檢驗,差異存在統(tǒng)計學意義(P0.05)。宮頸癌中SALL4蛋白的表達水平顯著高于其他三種宮頸組織(P0.05);低級別期的CIN組織中SALL4含量低于高級別期(P0.05);而正常宮頸組織中的SALL4表達水平在四種宮頸組織中表達水平最低(P0.05)。Bmi-1陽性表達率:正常宮頸組織、低級別宮頸上皮內(nèi)病變、高級別宮頸上皮內(nèi)病變和宮頸癌組織中的表達分別是6.67%、23.33%、46.67%、73.33%(P0.05);采用SPSS19.0統(tǒng)計軟件進行組間卡方檢驗,差異存在統(tǒng)計學意義(p0.05)。宮頸癌中Bmi-1的表達水平明顯高于CIN及正常宮頸組織(P0.05),高級別期CIN的Bmi-1表達明顯高于低級別期組織(P0.05),而CIN中Bmi-1的表達亦顯著高于正常宮頸組織(P0.05)。采用SPSS19.0統(tǒng)計軟件對SALL4與Bmi-1 pearson相關性檢驗,呈正相關(r=0.798,P0.01)。SALL4和Bmi-1與宮頸癌臨床病理特征關系:SALL4、Bmi-1蛋白表達水平與淋巴結轉移(t=1.907,p0.05),組織學分級(t=0.993,p0.05)及臨床分期(t=1.080,p0.05)相關,而與年齡(t=1.436,p0.05)、腫瘤大小(t=1.286,p0.05)均無關。SALL4、Bmi-1蛋白在40歲到59歲陽性率表達最高,但不具有統(tǒng)計學意義。臨床分期越高SALL4、Bmi-1蛋白表達水平越高。有淋巴結轉移的SALL4、Bmi-1蛋白表達明顯高于無淋巴結轉移的宮頸癌組織。結論:(1)在宮頸癌中SALL4和Bmi-1表達明顯升高,且高于其他宮頸對照組織(低級別宮頸上皮內(nèi)病變、高級別宮頸上皮內(nèi)病變、和正常宮頸組織),SALL4和Bmi-1與CIN病變級別相關,病變級別越高,表達越高,癌變的程度越大。(2)在宮頸癌的發(fā)病過程中SALL4和Bmi-1變化具有平行性,提示二者具有協(xié)同作用。
[Abstract]:Objective to investigate the significance of expression of B-cell specific moloney leukemia virus insertion site 1 (B-cell specific moloney leukemia virus insertion site 1) in carcinogenesis and progression of cervical carcinoma.Methods patients with cervical cancer, cervical intraepithelial lesions (CINs) and normal cervical tissues were selected from September 2014 to September 2016.Thirty cases of cervical cancer were randomly selected, including 22 cases of squamous cell carcinoma and 8 cases of adenocarcinoma. The age was 2569 years old with a median age of 45 years.Sixty patients with CIN were selected, including 30 cases of low grade CIN and 30 cases of high grade CIN. The age was 22 ~ 68 years old with a median age of 46 years.At the same time, 30 cases of normal cervical tissue were collected as control group.All cervical tissue specimens in this study were confirmed by pathology after operation and none of them received radiotherapy chemotherapy or any hormone therapy before operation.The expression of SALL4 and Bmi-1 in cervical tissues of the three groups were detected by immunohistochemical technique and SP method.SPSS19.0 statistical software was used to analyze the data. The data concentration trend and discrete trend (`X 鹵S) indicated that P0.05 was regarded as the standard of statistical significance.The correlation between the expression of SALL4 and Bmi-1 was analyzed by chi-square test.Results the positive expression rate of SALL4 in normal cervix, low grade cervical intraepithelial lesion, high grade cervical intraepithelial lesion and cervical carcinoma was 10.00 ~ 36.67 ~ 36.67 ~ (33.33) ~ 86.67 ~ (th) P 0.05 respectively.The difference was statistically significant (P 0.05).The expression of SALL4 protein in cervical cancer was significantly higher than that in the other three cervical tissues (P0.05A), the SALL4 level in low-grade CIN tissues was lower than that in high-grade CIN tissues, and the SALL4 expression level in normal cervix tissues was higher in the four cervical tissues.The lowest positive rate of P0.05.
