【摘要】：目的：通過對甲狀腺乳頭狀癌和結節(jié)性甲狀腺腫中TRα1蛋白表達量的測定,進一步研究甲狀腺良惡性腫瘤的發(fā)病機制,并為其臨床診斷、疾病鑒別、預后判斷以及治療方法提供指導。方法：選取新疆醫(yī)科大學附第五附屬醫(yī)院經(jīng)病理證實的2010年1月-2014年12月甲狀腺乳頭狀癌及結節(jié)性甲狀腺腫癌患者石蠟標本各50例,應用免疫組化(SP)法分別檢測甲狀腺乳頭狀癌和結節(jié)性甲狀腺腫患者腫瘤組織中TRα1蛋白的表達以及伴淋巴結轉移與無淋巴結轉移甲狀腺乳頭狀癌組織中TRα1蛋白的表達。結果：(1)TRα1蛋白陽性表達主要位于細胞核與細胞漿中；(2)TRα1蛋白在甲狀腺乳頭狀癌陽性表達率為38.0%(19/50),結節(jié)性甲狀腺腫中陽性表達率為84.0%(42/50),兩者表達差異有統(tǒng)計學意義(P0.05)；(3)TRal蛋白在甲狀腺乳頭狀癌中的表達與性別、年齡、腫瘤直徑、病理分期差異均無統(tǒng)計學意義(P0.05)；而與淋巴結轉移具有統(tǒng)計學意義(P0.05)；TRα1蛋白在結節(jié)性甲狀腺腫中的表達與性別、年齡以及腫瘤直徑之間的差異均無統(tǒng)計學意義(P0.05)。結論：(1)TRa 1蛋白有助于甲狀腺乳頭狀癌與結節(jié)性甲狀腺腫的診斷與鑒別診斷；(2)TRα1蛋白對于甲狀腺乳頭狀癌的淋巴結轉移、臨床分期以及預后有預測作用。
[Abstract]:Objective: to study the pathogenesis of thyroid benign and malignant neoplasms by measuring the expression of tr 偽 1 protein in papillary thyroid carcinoma and nodular goiter.Prognostic judgment and treatment methods provide guidance.Methods: paraffin wax samples of 50 cases of papillary thyroid carcinoma and 50 cases of nodular goiter carcinoma confirmed by pathology in the Fifth affiliated Hospital of Xinjiang Medical University were selected.The expression of tr 偽 1 protein in thyroid papillary carcinoma and nodular goiter and the expression of tr 偽 1 protein in thyroid papillary carcinoma with and without lymph node metastasis were detected by immunohistochemical staining.Expression and sex of papillary thyroid carcinoma,There was no significant difference in age, tumor diameter and pathological stage (P 0.05), but there was no significant difference in the expression of tr 偽 1 protein with lymph node metastasis in nodular goiter and sex, the difference between age and tumor diameter was not significant (P 0.05).Conclusion the protein of TRA 1 protein is helpful to the diagnosis and differential diagnosis of papillary thyroid carcinoma and nodular goiter. TR 偽 1 protein can predict the lymph node metastasis, clinical stage and prognosis of papillary thyroid carcinoma.
