【摘要】：目的比較術(shù)前同期放化療與術(shù)前化療對(duì)局部晚期胃癌的療效及不良反應(yīng)。方法將局部晚期胃癌患者72例隨機(jī)分為:術(shù)前同期放化療組36例:順鉑+卡培他濱術(shù)前3個(gè)療程,并與放療同期治療;術(shù)前化療組36例:順鉑+卡培他濱術(shù)前3個(gè)療程。評(píng)價(jià)兩組的術(shù)后病理完全緩解(p CR)率、R0切除率及不良反應(yīng),并隨訪比較2年無進(jìn)展生存(PFS)和2年總生存(OS)。結(jié)果術(shù)前同期放化療組的p CR率、R0切除率和2年P(guān)FS都要優(yōu)于術(shù)前化療組(P=0.042、0.023、0.032);術(shù)前同期放化療組的2年OS與術(shù)前化療組差異無統(tǒng)計(jì)學(xué)意義(P=0.518);術(shù)前同期放化療組發(fā)生Ⅲ度腹痛、Ⅲ度惡心嘔吐、Ⅲ度白細(xì)胞下降與術(shù)前化療組差異無統(tǒng)計(jì)學(xué)意義(P=0.074、0.358、0.691)。結(jié)論局部晚期胃癌術(shù)前同期放化療比術(shù)前化療能夠獲得更好的p CR、R0切除和PFS,兩組不良反應(yīng)相近,但術(shù)前同期放化療不能延長OS。
[Abstract]:Objective to compare the efficacy and adverse effects of preoperative radiotherapy and chemotherapy on locally advanced gastric cancer. Methods 72 patients with locally advanced gastric cancer were randomly divided into preoperative radiotherapy and chemotherapy group (n = 36) and cisplatin capecitabine group (n = 36). 36 patients in the preoperative chemotherapy group were treated with cisplatin capecitabine for 3 courses before operation. Results the rate of pCR, R0 and PFS in the preoperative radiotherapy and chemotherapy group were higher than those in the preoperative chemotherapy group and the preoperative chemotherapy group, and the 2-year OS in the preoperative chemotherapy group and the preoperative chemotherapy group were higher than those in the preoperative chemotherapy group and the preoperative chemotherapy group respectively, and the two year OS in the preoperative radiotherapy and chemotherapy group was higher than that in the preoperative chemotherapy group and that in the preoperative chemotherapy group. There was no significant difference between the two groups, and the third degree abdominal pain occurred in the same time before the operation in the radiotherapy and chemotherapy group, and there was no significant difference between the two groups.
