【摘要】：目的:探討luminal B型晚期乳腺癌患者的臨床病理特征及預后因素。方法:收集2008年6月至2013年6月天津醫(yī)科大學腫瘤醫(yī)院收治的206例luminal B型晚期乳腺癌患者的臨床資料,回顧性分析其臨床病理特征及影響預后的因素。結(jié)果:206例患者中HER-2陽性型54例(26.2%),HER-2陰性型152例(73.8%)。其中HER-2陽性型患者中Ki-6730%為57.4%(31/54),較HER-2陰性型的55.9%(85/152)高,差異具有統(tǒng)計學意義(P0.01);HER-2陽性型患者中無病生存期(disease free survival,DFS)36個月為79.6%(43/54),較HER-2陰性型的65.1%(99/152)高,差異具有統(tǒng)計學意義(P0.05)。206例患者的中位生存期為25(2.1~85.0)個月,是否合并內(nèi)臟轉(zhuǎn)移、一線解救化療療效、是否行解救內(nèi)分泌治療是luminal B型晚期乳腺癌患者的獨立預后因素(P0.05)。結(jié)論:luminal B型晚期乳腺癌患者中,HER-2陽性型中的Ki-67指數(shù)比HER-2陰性型更高,且更易在3年內(nèi)發(fā)生復發(fā)、轉(zhuǎn)移。合并內(nèi)臟轉(zhuǎn)移、一線解救化療時病情進展、未行解救內(nèi)分泌治療是影響luminal B型晚期乳腺癌患者不良預后的獨立因素。
[Abstract]:Objective: to investigate the clinicopathological features and prognostic factors of luminal B type advanced breast cancer.Methods: the clinical data of 206 patients with advanced luminal B breast cancer admitted to Tianjin Medical University Cancer Hospital from June 2008 to June 2013 were retrospectively analyzed and their clinicopathological features and prognostic factors were analyzed retrospectively.Results among the 206 patients, 54 were HER-2 positive and 152 were HER-2 negative.The difference was statistically significant in the median survival time of P0.05. 206 patients with advanced luminal B breast cancer. Whether the patients had visceral metastasis, first-line chemotherapeutic effect and endocrine rescue therapy was the independent prognostic factor of luminal B type advanced breast cancer.Conclusion the Ki-67 index of HER-2 positive type is higher than that of HER-2 negative type in patients with advanced breast cancer of type B, and it is more likely to recur and metastasize within 3 years.The patients with visceral metastasis, first-line rescue chemotherapy and no rescue endocrine therapy were independent factors influencing the poor prognosis of patients with luminal B type advanced breast cancer.
