【摘要】：目的:通過收集食管癌術(shù)后局部復(fù)發(fā)患者的臨床資料,從獲得的各種數(shù)據(jù)進(jìn)行統(tǒng)計(jì)學(xué)分析,找出局部復(fù)發(fā)的規(guī)律以及術(shù)后復(fù)發(fā)影響預(yù)后的危險(xiǎn)因素。方法:收集河北醫(yī)科大學(xué)第四醫(yī)院2008-2015年在我院行食管癌根治術(shù)(術(shù)前經(jīng)CT、MRI、超聲、骨掃描等相關(guān)檢查均未發(fā)現(xiàn)遠(yuǎn)處轉(zhuǎn)移),術(shù)后發(fā)生局部復(fù)發(fā)、轉(zhuǎn)移并且就診于我院放射治療科行放療的病人共706例的臨床資料。局部復(fù)發(fā)包括:鎖骨上淋巴結(jié)轉(zhuǎn)移、上縱膈淋巴結(jié)轉(zhuǎn)移、中下縱膈淋巴結(jié)轉(zhuǎn)移、吻合口復(fù)發(fā)或者食管原瘤床復(fù)發(fā)、腹腔淋巴結(jié)轉(zhuǎn)移。術(shù)后淋巴結(jié)復(fù)發(fā)的診斷標(biāo)準(zhǔn)總結(jié)如下:1、經(jīng)過穿刺得到病理學(xué)或細(xì)胞學(xué)上的診斷;2、影像學(xué)中的診斷:一般認(rèn)為可見的淋巴結(jié)短徑≥10mm可認(rèn)為是區(qū)域淋巴結(jié)復(fù)發(fā)。吻合口復(fù)發(fā)診斷標(biāo)準(zhǔn):食管鏡下可見新生物及咬檢病理證實(shí)或者CT以及PET-CT可見不規(guī)則軟組織腫塊以及異常高代謝區(qū)。采用軟件spss21.0進(jìn)行統(tǒng)計(jì)學(xué)分析,生存時(shí)間為從手術(shù)時(shí)間到死亡時(shí)間或最后一次隨訪時(shí)間,計(jì)數(shù)資料采用?2檢驗(yàn),生存分析采用Kaplan-Meier法,行Log-rank檢驗(yàn),多因素預(yù)后分析采用Cox模型,P
[Abstract]:Objective: to collect the clinical data of patients with local recurrence of esophageal cancer after operation, and to analyze the data obtained statistically. To find out the regularity of local recurrence and the risk factors of postoperative recurrence affecting prognosis.
