胃癌外周血微小RNA-23a表達(dá)及其與臨床特征相關(guān)性分析

【摘要】:背景及目的 胃癌是危害我國(guó)人民身體健康的重大疾病。胃癌防控的關(guān)鍵在于早期診斷和早期治療。目前胃癌的篩查和早期診斷依賴(lài)胃鏡檢查,費(fèi)用高,患者接受度低。缺乏用于胃癌診斷外周血分子標(biāo)志物。本實(shí)驗(yàn)室前期關(guān)于外周血miRNA高通量測(cè)序的研究發(fā)現(xiàn)多個(gè)在胃癌患者中和健康人群中差異表達(dá)的miRNA,包其中括miR-23a.本研究旨在不同人群中收集外周血,分析其miR-23a表達(dá)量,以期發(fā)現(xiàn)其表達(dá)量與胃癌的關(guān)系以及與臨床和病理特征的相關(guān)性,進(jìn)而評(píng)估其是否有成為胃癌外周血檢測(cè)分子標(biāo)志物的可能性。 方法 在通過(guò)醫(yī)院倫理委員會(huì)批準(zhǔn)以及患者知情同意的情況下收集邵逸夫醫(yī)院就診的患者外周血及臨床資料,采用高速離心的方式獲取血清,后通過(guò)實(shí)時(shí)熒光定量逆轉(zhuǎn)錄PCR的方法檢測(cè)血清中miR-23a表達(dá)量,結(jié)合檢測(cè)結(jié)果及臨床資料,采用獨(dú)立樣本的t檢驗(yàn)、卡方檢驗(yàn)及spearman相關(guān)性分析其與胃癌的相關(guān)性。 結(jié)果 1)胃癌組外周血miR-23a表達(dá)量較健康對(duì)照組顯著升高,差異具有統(tǒng)計(jì)學(xué)意義。 2)以外周血miR-23a表達(dá)量ACT值12.335為陽(yáng)性判斷標(biāo)準(zhǔn)時(shí),對(duì)于胃癌的判斷靈敏度為63%,特異度為60%。胃癌組中陽(yáng)性患者淋巴結(jié)轉(zhuǎn)移率較高,與陰性者比較有統(tǒng)計(jì)學(xué)意義。 3)在胃癌患者中外周血1niR-23a表達(dá)量與印戒細(xì)胞癌呈負(fù)相關(guān),及腫瘤的分化程度呈正相關(guān)。 結(jié)論 外周血miR-23a作為胃癌診斷的外周血分子標(biāo)志物可能有有助于胃癌患者預(yù)后的判斷。
[Abstract]:Background and purpose. Gastric cancer is a major disease that endangers the health of our people. The key to the prevention and control of gastric cancer lies in early diagnosis and treatment. At present, the screening and early diagnosis of gastric cancer depends on gastroscopy and the cost is high. Low acceptance of patients. Lack of molecular markers for the diagnosis of gastric cancer peripheral blood. Our previous study of peripheral blood miRNA high-throughput sequencing found a number of differentially expressed miRNAs in patients with gastric cancer and in healthy people, including. The aim of this study was to collect peripheral blood from different populations. The expression of miR-23a was analyzed in order to find out the relationship between the expression of miR-23a and gastric cancer and its correlation with clinical and pathological features, and to evaluate the possibility of its expression as a molecular marker in peripheral blood detection of gastric cancer. Method. With the approval of the Hospital Ethics Committee and the informed consent of the patients, the peripheral blood and clinical data of the patients treated by Shaw Hospital were collected, and the serum was obtained by high-speed centrifugation. The expression of miR-23a in serum was detected by real-time fluorescence quantitative reverse transcription (PCR). The correlation between miR-23a and gastric cancer was analyzed by t-test of independent samples, chi-square test and spearman correlation analysis. Results. 1) the expression of miR-23a in peripheral blood of gastric cancer group was significantly higher than that of healthy control group. 2) when the ACT value of miR-23a expression in peripheral blood was taken as the positive criterion, the sensitivity and specificity for gastric cancer were 63 and 600.The lymph node metastasis rate of the positive patients was higher than that of the negative patients. 3) in gastric cancer patients, the expression of 1niR-23a in peripheral blood was negatively correlated with signet ring cell carcinoma and positively correlated with the degree of tumor differentiation. Conclusion. Peripheral blood miR-23a as a molecular marker in the diagnosis of gastric cancer may be helpful to the prognosis of gastric cancer patients.
