【摘要】：目的探討將肺泡灌洗液中肺特異性蛋白LUNXm RNA用于輔助肺癌診斷及輔助中醫(yī)辨證的可行性;評(píng)價(jià)外周血肺特異性蛋白LUNXm RNA作為肺癌微轉(zhuǎn)移的特異性標(biāo)志物的價(jià)值及其與中醫(yī)證型的相關(guān)性方法對(duì)接受纖維支氣管鏡的病例采集支氣管肺泡灌洗液標(biāo)本,對(duì)符合條件的病例采集外周血標(biāo)本,提取標(biāo)本中的總RNA,加入含目的基因引物的反轉(zhuǎn)錄系統(tǒng)反轉(zhuǎn)錄生成c DNA,運(yùn)用RT-PCR方法對(duì)肺泡灌洗液和外周血的肺特異性蛋白LUNXm RNA進(jìn)行檢測(cè),比較肺泡灌洗液的肺特異性蛋白LUNXm RNA在肺癌患者與非肺癌患者之間的表達(dá)差異,并分析檢測(cè)值在肺癌不同證型之間的分布特點(diǎn);分析外周血的陽(yáng)性檢出率與肺癌TNM分期的關(guān)系及其在肺癌中醫(yī)證型間的分布規(guī)律。結(jié)果肺癌組和結(jié)核組患者肺泡灌洗液中肺特異性蛋白LUNXm RNA的表達(dá)水平存在統(tǒng)計(jì)學(xué)差異(結(jié)核組檢測(cè)值高于肺癌組),各證型間肺泡灌洗液LUNXm RNA表達(dá)水平有顯著差異,其中痰濕內(nèi)盛證、陰虛毒熱證表達(dá)含量最高(兩者之間差異無(wú)統(tǒng)計(jì)學(xué)意義P0.05),氣滯血瘀證次之,氣陰兩虛證最低;在肺癌III期、IV期及肺癌陰虛毒熱證、氣陰兩虛證中外周血LUNXm RNA有較高的陽(yáng)性檢出率,且肺癌各證型間外周血LUNXm RNA陽(yáng)性檢出率差異具有統(tǒng)計(jì)學(xué)意義(P均0.05)。結(jié)論肺泡灌洗液LUNXm RNA輔助肺癌診斷不具備特異性,但與肺癌中醫(yī)辨證分型有關(guān),多與痰、熱、毒等病理因素有關(guān);外周血LUNXm RNA可用于檢測(cè)肺癌微轉(zhuǎn)移,但靈敏度欠佳,與肺癌中醫(yī)證型有關(guān),在陰虛毒熱、氣陰兩虛等虛證證型中檢出率高。
[Abstract]:Objective to explore the feasibility of using lung specific protein (LUNXm RNA) in alveolar lavage fluid to assist lung cancer diagnosis and TCM syndrome differentiation. To evaluate the value of peripheral blood lung specific protein LUNXm RNA as a specific marker for micrometastasis of lung cancer and its correlation with TCM syndromes the bronchoalveolar lavage fluid (BALF) samples were collected from patients receiving fiberoptic bronchoscopy. Peripheral blood samples were collected from eligible patients. Total RNAs were extracted from the samples. Reverse transcription system containing target gene primers was added to reverse transcription system to produce c DNA. the lung specific protein LUNXm RNA in alveolar lavage fluid and peripheral blood was detected by RT-PCR method. To compare the expression of lung specific protein LUNXm RNA in alveolar lavage fluid between lung cancer patients and non-lung cancer patients, and to analyze the distribution characteristics of the detection values among different types of lung cancer. To analyze the relationship between the positive detection rate of peripheral blood and TNM stage of lung cancer and its distribution in TCM syndrome of lung cancer. Results the expression level of lung specific protein LUNXm RNA in alveolar lavage fluid of lung cancer patients and tuberculosis patients was statistically analyzed. There were significant differences in the expression of LUNXm RNA in alveolar lavage fluid between different syndromes, the detection value in tuberculosis group was higher than that in lung cancer group. The expression of phlegm and dampness and yin deficiency and toxin heat syndrome were the highest (there was no significant difference between the two groups (P 0.05), Qi stagnation and blood stasis syndrome was the second, qi and yin deficiency syndrome was the lowest, in III stage IV of lung cancer and yin deficiency heat syndrome of lung cancer, there was no significant difference between them. The positive rate of LUNXm RNA in peripheral blood of Qi-Yin deficiency syndrome was higher, and the positive rate of LUNXm RNA in peripheral blood was significantly different among different types of lung cancer (P < 0.05). Conclusion LUNXm RNA in alveolar lavage fluid is not specific in the diagnosis of lung cancer. However, LUNXm RNA in peripheral blood can be used to detect micrometastasis of lung cancer, but its sensitivity is not good, and it is related to TCM syndrome type of lung cancer. The positive rate of deficiency syndrome such as deficiency of qi and yin is high.
