【摘要】：目的比較腹腔鏡輔助下與開腹保肛根治術(shù)對超低位直腸癌的治療效果。方法超低位直腸癌患者133例,其中行開腹保肛根治術(shù)者62例(開腹組),腹腔鏡輔助下實施保肛根治術(shù)者71例(腹腔鏡組),對比兩組手術(shù)情況、并發(fā)癥以及肛管功能。結(jié)果與開腹組比較,腹腔鏡組手術(shù)時間長、術(shù)中出血量少,術(shù)后肛門開始排氣、開始飲食、住院時間短(P均0.05)。術(shù)后1年,兩組肛腸壓力均較術(shù)前下降,以腹腔鏡組為著(P均0.05)。與開腹組比較,腹腔鏡組優(yōu)良率高,Wexner便秘評分低,術(shù)后總并發(fā)癥率低(P均0.05)。兩組局部復(fù)發(fā)率、遠(yuǎn)處轉(zhuǎn)移率、病死率比較,P均0.05。結(jié)論腹腔鏡下保肛根治術(shù)治療超低位直腸癌,在臨床效果方面和開腹保肛根治術(shù)相當(dāng),但術(shù)中創(chuàng)傷小、術(shù)后患者恢復(fù)快、并發(fā)癥發(fā)生率低,且肛管功能優(yōu)。
[Abstract]:Objective to compare the curative effect of laparoscopic assisted and open anal preserving radical resection for ultralow rectal cancer. Methods 133 cases of ultralow rectal cancer were treated. Among them, 62 cases (laparotomy group) and 71 cases (laparoscopically assisted radical anus preserving operation group) were treated with open anus preserving radical operation (Laparoscopic group), the operation condition, complications and anal canal function were compared between the two groups, the results were compared with that in the laparotomy group. In the laparoscopy group, the operation time was longer, the amount of bleeding was less during the operation, the anus began to exhaust after operation, the beginning of diet and the hospitalization time were 0.050.The pressure of the anus and intestine in the two groups was lower than that of the preoperation one year after operation, and the laparoscopy group was 0.050.Compared with the open group, the anus and intestine pressure of the two groups decreased. The excellent and good rate of laparoscopy group was high and the score of Wexner constipation was low, and the total postoperative complication rate was lower than 0.050.The local recurrence rate, distant metastasis rate and mortality rate of the two groups were 0.05.Conclusion Laparoscopic sphincter preserving radical resection is effective in the treatment of ultralow rectal cancer. The clinical effect was similar to that of radical operation, but the wound was small, the postoperative patients recovered quickly, the incidence of complications was low, and the anal canal function was excellent.
