【摘要】：目的監(jiān)測(cè)胃癌患者外周血游離DNA(cell-free DNA,cf-DNA)的含量,探討其在胃癌診治和預(yù)后中的價(jià)值。方法收集81例原發(fā)胃癌、18例胃良性腫瘤、19例胃癌前病變、43例同期門(mén)診健康體檢者以及部分術(shù)后和隨訪的外周血標(biāo)本,提取游離DNA,熒光定量PCR檢測(cè)其含量(ALU115和ALU247)并計(jì)算完整性(ALU247/ALU115),分析血清游離DNA和血漿游離DNA含量之間的相關(guān)性。結(jié)果血漿游離DNA含量和完整性在四組之間的差異有統(tǒng)計(jì)學(xué)意義(ALU115、ALU247以及完整性的P值均0.001),術(shù)前術(shù)后的差異也有統(tǒng)計(jì)學(xué)意義(P值分別為0.000、0.013、0.010),隨訪前后完整性差異有統(tǒng)計(jì)學(xué)意義(P=0.029),在有無(wú)轉(zhuǎn)移、分期、分化程度間差異有統(tǒng)計(jì)學(xué)意義(P值分別為0.013、0.042、0.003)。血漿和血清游離DNA含量具有相關(guān)性且血漿離散趨勢(shì)更小。ALU247的ROC曲線(xiàn)下面積最高(0.985)。結(jié)論游離DNA檢測(cè)在胃癌的診治和預(yù)后中有一定的價(jià)值。
[Abstract]:The purpose of monitoring patients with gastric cancer peripheral blood free DNA (cell-free DNA, cf-DNA) content, and explore the diagnosis and treatment and prognosis of gastric carcinoma. Methods 81 cases of primary gastric carcinoma, 18 cases of gastric benign tumors, 19 cases of precancerous lesions, 43 cases of healthy year outpatient service and part of the postoperative and follow-up peripheral blood samples, extract free DNA, fluorescence quantitative PCR to detect the content of (ALU115 and ALU247) and integrity (ALU247/ALU115), analyze the correlation between the content of free DNA in serum free DNA and plasma. Results there were significant differences in plasma free DNA content and integrity between the four groups (ALU115, ALU247 and integrity the P value was 0.001), preoperative postoperative difference was statistically significant (P = 0.000,0.013,0.010), statistically significant difference before and after complete follow-up (P=0.029), in metastasis, staging, there was statistically significant difference between the degree of differentiation Meaning (P = 0.013,0.042,0.003). The free DNA content in plasma and serum is correlated and the plasma dispersion trend is smaller. The area under the ROC curve of.ALU247 is the highest (0.985). Conclusion free DNA detection has certain value in the diagnosis and prognosis of gastric cancer.
