【摘要】：乳腺癌是全球女性惡性腫瘤發(fā)病率最高的疾病,對(duì)適齡婦女進(jìn)行群體性的乳腺癌篩查旨在早發(fā)現(xiàn)、早診斷、早治療,從而有效降低乳腺癌的死亡率。歐美國家于數(shù)十余年前開始全國范圍的乳腺癌的群體篩查,早期的觀察性研究認(rèn)為篩查能獨(dú)立降低乳腺癌的死亡率,而近期的許多群體研究結(jié)果卻不斷對(duì)鉬靶篩查的獲益提出質(zhì)疑。中國對(duì)腫瘤的篩查早已有之,包括乳腺癌在各地區(qū)的群體性篩查也開展多年,但尚無一個(gè)權(quán)威性的研究結(jié)論供我們參考。筆者借鑒國內(nèi)外的相關(guān)研究結(jié)果和自己在臨床工作中的機(jī)會(huì)性篩查體會(huì),對(duì)中國乳腺癌篩查的相關(guān)問題進(jìn)行了思考,提出群體性的鉬靶篩查并不適合中國國情,不應(yīng)予以提倡和推廣。應(yīng)當(dāng)建立符合中國乳腺癌發(fā)病特征的風(fēng)險(xiǎn)評(píng)估模型,根據(jù)風(fēng)險(xiǎn)評(píng)估結(jié)果,有針對(duì)性地對(duì)高危女性進(jìn)行重點(diǎn)篩查,結(jié)合機(jī)會(huì)性篩查,采取臨床觸診、彩超和鉬靶相互結(jié)合為主的篩查方法,才是適合中國國情的乳腺癌篩查之道。
[Abstract]:Breast cancer is the disease with the highest incidence of malignant tumors in women in the world. Screening for breast cancer among women of appropriate age aims at early detection, early diagnosis and early treatment. In order to effectively reduce the mortality rate of breast cancer, European and American countries began nationwide breast cancer screening more than a few decades ago. Early observational studies suggested that screening can independently reduce breast cancer mortality. Many recent studies have questioned the benefits of mammography screening, which has long been available in China, including mass screening for breast cancer in various regions for many years. However, there is not an authoritative conclusion for our reference. The author, referring to the relevant research results at home and abroad and his own experience of opportunistic screening in clinical work, thinks about the relevant problems of breast cancer screening in China. It is suggested that mass mammography screening is not suitable for China's national conditions and should not be promoted and popularized. A risk assessment model should be established in accordance with the characteristics of breast cancer incidence in China, and according to the risk assessment results, Targeted screening of high-risk women, combined with opportunistic screening, clinical palpation, color ultrasound and molybdenum target, is the best way to screen breast cancer in China.
