【摘要】：目的:1、探討表觀彌散系數(shù)(ADC值)及時間-信號強度曲線參數(shù)(TIC參數(shù):T_(peak)、SI_(peak)、E_(max))聯(lián)合運用在直腸癌術(shù)前診斷中的應(yīng)用價值。2、通過研究ADC值與直腸腺癌分化程度的相關(guān)性,分析ADC值在直腸癌分級中的應(yīng)用價值。3、探究TIC參數(shù)(T_(peak)、SI_(peak)、E_(max))與腫瘤分級的關(guān)系,分析TIC參數(shù)在直腸癌分級中的應(yīng)用價值。材料與方法:回顧性分析山西醫(yī)學(xué)科學(xué)院山西大醫(yī)院2015年1月至2017年1月直腸腺癌58例患者及32例正常人的DWI圖像特點,術(shù)后病理顯示高分化腺癌18例,中分化腺癌26例,低分化腺癌14例,經(jīng)Philips Achieva 3.0TMR工作站測量其在b值為800s/mm~2時平均ADC值,比較不同分組間差異有無統(tǒng)計學(xué)意義并分析與腫瘤分化程度的相關(guān)性,其中36例進行DCE-MRI掃描,其中高分化腺癌10例,中分化腺癌18例,低分化腺癌8例,直腸正常腸壁組36例,在工作站繪制TIC曲線并記錄TIC參數(shù)(T_(peak)、SI_(peak)、E_(max)),比較不同分組間差異有無統(tǒng)計學(xué)意義并分析TIC參數(shù)與分化程度的關(guān)系。結(jié)果:高分化組平均ADC值為(0.51±0.02)×10~(-3)mm~2/s,中分化組平均ADC值為(0.47±0.02)×10~(-3)mm~2/s,低分化組平均ADC值為(0.40±0.03)×10~(-3)mm~2/s,三組平均ADC值有明顯差異(P0.05),ADC值與腫瘤分化惡性程度呈負(fù)相關(guān),相關(guān)系數(shù)為Spearmanr=-0.864(P0.01);直腸腺癌組平均ADC值為(0.46±0.05)×10~(-3)mm~2/s,正常組平均ADC為(0.53±0.01)×10~(-3)mm~2/s,直腸腺癌組與正常組平均ADC值有明顯差異(P0.05);直腸腺癌組平均T_(peak)、SI_(peak)、E_(max)分別為83.88±15.75、3620.85±131.21、2591.57±90.53,直腸正常腸壁組T_(peak)、SI_(peak)、E_(max)分別為204.70±29.48、1290.52±237.49、952.96±222.26,直腸腺癌組與直腸正常腸壁組T_(peak)、SI_(peak)、E_(max)有明顯差異(P0.05);高分化組、中分化組、低分化組的T_(peak)、SI_(peak)、E_(max)見表5,T_(peak)與直腸腺癌分化程度呈正相關(guān),相關(guān)系數(shù)為Spearmanr=0.526(P0.05),高低分化及中低分化組平均T_(peak)差異具有統(tǒng)計學(xué)意義(P0.05),SI_(peak)、E_(max)與腫瘤分化程度無相關(guān)性,但隨著分化程度減低,有增高趨勢;直腸腺癌TIC曲線為流出型或平臺型,正常組TIC曲線為流入型。結(jié)論:1、ADC值及TIC參數(shù)(T_(peak)、SI_(peak)、E_(max))對直腸腺癌有診斷價值。2、ADC值可以反應(yīng)直腸腺癌的分化程度,T_(peak)在一定程度上可以反應(yīng)直腸腺癌分化程度,但不具有有效性,SI_(peak)、E_(max)能間接反映腫瘤生物學(xué)信息,但具有局限性;TIC參數(shù)與直腸腺癌分化程度的關(guān)系還需進一步研究。
