【摘要】：目的 評(píng)價(jià)腹腔鏡腹膜外前列腺癌根治術(shù)(ELRP)的臨床療效并總結(jié)手術(shù)經(jīng)驗(yàn)。方法 回顧性分析2010年1月至2015年6月我院同一術(shù)者完成的50例ELRP手術(shù)患者的臨床資料。結(jié)果 50例均在腹腔鏡下順利完成手術(shù),平均手術(shù)時(shí)間238.8min,平均手術(shù)出血量409.1 m L,術(shù)中或術(shù)后輸血6例(12%);術(shù)后平均留置導(dǎo)尿時(shí)間23.7 d;術(shù)后平均住院15 d。術(shù)后恢復(fù)進(jìn)食時(shí)間2~4 d,離床活動(dòng)時(shí)間1~3 d。術(shù)后淋巴結(jié)陽(yáng)性3例(6%),切緣陽(yáng)性7例(14%)。出現(xiàn)手術(shù)相關(guān)并發(fā)癥共9例(18%)。術(shù)后隨訪6~58個(gè)月,平均12.5個(gè)月。生化復(fù)發(fā)1例(2%),術(shù)后無(wú)瘤生存率達(dá)84%。至隨訪終點(diǎn)未出現(xiàn)尿失禁病例,尿控率達(dá)100%。結(jié)論 ELRP的臨床應(yīng)用安全有效,隨著微創(chuàng)技術(shù)的發(fā)展,手術(shù)適應(yīng)證越來(lái)越廣泛,但對(duì)于高危前列腺癌患者術(shù)后的腫瘤學(xué)效果仍需大樣本及長(zhǎng)時(shí)間隨訪研究。
[Abstract]:Objective to evaluate the clinical effect of laparoscopic extraperitoneal prostatectomy for prostatic cancer and summarize the experience of the operation. Methods the clinical data of 50 cases of ELRP operation performed in our hospital from January 2010 to June 2015 were analyzed retrospectively. Results 50 cases were successfully operated under laparoscope. The mean operative time was 238.8 min, the average amount of operative bleeding was 409.1 mL, the blood transfusion was 12 cases during or after operation, the mean time of indwelling urethral catheterization after operation was 23.7 days, the average hospitalization time was 15 days, the recovery time of food was 24 days, the time of moving from bed was 1 and 3 days after operation. 3 cases were positive and 7 cases were positive in incisal margin, 9 cases had complications associated with operation. The follow-up was 6 ~ 58 months after operation. The mean time was 12. 5 months. One case of biochemical recurrence had a tumor free survival rate of 84. To the end of follow-up, there were no urinary incontinence cases, and the urinary control rate was 100. Conclusion the clinical application of ELRP is safe and effective. With the development of minimally invasive technique, the indication of operation is more and more extensive. However, a large sample and long-term follow-up study is needed for postoperative oncology in high-risk prostate cancer patients.
