【摘要】：目的:探討多層CT(MSCT)三期動(dòng)態(tài)增強(qiáng)掃描對(duì)腎透明細(xì)胞癌(CCRCC)、嫌色細(xì)胞癌(CRCC)及嗜酸細(xì)胞腺瘤(OA)的鑒別診斷價(jià)值。方法:采用雙盲法回顧性分析經(jīng)手術(shù)及病理證實(shí)的55例CCRCC、16例CRCC及9例OA患者的CT圖像,比較三者在腫瘤大小、性質(zhì)(實(shí)性、囊實(shí)性及囊性)、校正強(qiáng)化CT值以及強(qiáng)化形式(均勻強(qiáng)化、不均勻強(qiáng)化及周邊強(qiáng)化)等方面的差異。結(jié)果:(1)三組腫瘤最大徑無顯著性差異。(2)CRCC以實(shí)性為主,CCRCC以囊實(shí)性為主,OA實(shí)性與囊實(shí)性所占比例相仿,三者間有顯著性差異(χ~2=14.10,P0.001)。(3)皮髓質(zhì)期CCRCC校正強(qiáng)化CT值高于CRCC及OA(P0.001),而CRCC與OA間無顯著性差異(P0.05);實(shí)質(zhì)期CCRCC校正強(qiáng)化CT值高于CRCC(P0.001),而CCRCC與OA及CRCC與OA間無顯著性差異(P0.05),OA校正強(qiáng)化CT值介于CCRCC與CRCC之間;排泄期CRCC校正強(qiáng)化CT值低于CCRCC與OA(P0.05),而CCRCC與OA間無顯著性差異(P0.05)。(4)強(qiáng)化形式比較,CCRCC與CRCC及CCRCC與OA間差異具有統(tǒng)計(jì)學(xué)意義(χ~2=15.89,P0.001和χ~2=11.15,P0.05),而CRCC與OA間差異無統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:腫瘤的校正強(qiáng)化CT值是鑒別腎腫瘤類型的一個(gè)重要指標(biāo),而腫瘤的成分性質(zhì)及強(qiáng)化形式也具有一定的鑒別意義。
[Abstract]:Objective: to evaluate the differential diagnosis of CCRCCT, chromophobe cell carcinoma (CRCCCC) and eosinophil adenoma (OAA) with multilayer CTT MSCT in 55 cases of renal clear cell carcinoma (CCRCC). Methods: 55 cases proved by operation and pathology were retrospectively analyzed by double blind method. Ct images of 16 patients with CRCC and 9 patients with OA. The size, nature (solid, cystic, and cystic) of the tumor, the corrected CT value and the enhancement form (uniform enhancement) were compared among the three groups. Results there was no significant difference in the maximum diameter of tumor in the three groups. The majority of CCRCC was solid, and the proportion of OA solid and cystic solid was similar to that of CCRCC. There was significant difference among the three groups (蠂 ~ (2) P ~ (14. 10) P 0.001 ~ (-1)). The CT value of CCRCC correction in medullary phase was higher than that of CRCC and OAP 0.001, but there was no significant difference between CRCC and OA (P 0.05). The CT value of CCRCC correction in substantive phase was higher than that of CCRCC (P 0.001), but there was no significant difference between CCRCC and OA and CRCC and OA (P 0.05). The value of CT was between CCRCC and CRCC. The CT value of CRCC correction enhancement in excretory phase was lower than that of CCRCC and OAA P0.05, but there was no significant difference between CCRCC and OA. Conclusion: there was significant difference between CCRCC and CRCC and between CCRCC and OA (蠂 2 + 15.89%, P 0.001, 蠂 ~ 2 + 11.15 P 0.05), but there was no significant difference between CRCC and OA (P 0.05). The corrected enhanced CT value of tumor is an important index to distinguish the type of renal tumor. The character of tumor components and the enhancement form also have certain differentiation significance.
