【摘要】：目的:綜合評(píng)價(jià)及探討染發(fā)劑使用與非霍奇金淋巴瘤相關(guān)發(fā)病風(fēng)險(xiǎn)的關(guān)系,以豐富對(duì)非霍奇金淋巴瘤病因?qū)W和流行病學(xué)的認(rèn)識(shí),為篩選和保護(hù)易感人群提供參考依據(jù),同時(shí)為臨床流行病預(yù)防工作開(kāi)展提供依據(jù)。方法:以中文“非霍奇金淋巴瘤”、“淋巴瘤”、“染發(fā)劑”、“病例對(duì)照和(或)隊(duì)列研究”、“危險(xiǎn)因素”,英文以“(non-Hodgkin lymphoma OR Lymphoma OR NHL)AND(hair dye OR hair colorants OR hair coloring OR hair coloring agent OR hair color/colour)AND(risk OR incidence)AND(cohort OR case-control study)”以及同義詞等為關(guān)鍵詞,分別在中國(guó)學(xué)術(shù)期刊全文數(shù)據(jù)庫(kù)、萬(wàn)方全文數(shù)據(jù)庫(kù)和PubMed,the Cochrane Library以及Embase數(shù)據(jù)庫(kù)進(jìn)行檢索,同時(shí)進(jìn)行附加檢索及手工檢索補(bǔ)充,收集已發(fā)表的病例-對(duì)照研究。依據(jù)文獻(xiàn)納入標(biāo)準(zhǔn)篩選、納入合格的文獻(xiàn),按照NOS量表對(duì)納入文獻(xiàn)的質(zhì)量進(jìn)行全面評(píng)價(jià)并提取相關(guān)資料。納入以O(shè)R值及95%可信區(qū)間評(píng)價(jià)染發(fā)劑使用與NHL關(guān)系的病例對(duì)照研究及隊(duì)列研究。納入文獻(xiàn)使用I2進(jìn)行異質(zhì)性,漏斗圖Egger’s檢驗(yàn)發(fā)表文獻(xiàn)是否存在偏倚,應(yīng)用RevMan 5.2軟件和Stata 12.0軟件對(duì)數(shù)據(jù)分析。結(jié)果:最終納入16篇文章,其中13篇病例對(duì)照研究,3篇項(xiàng)隊(duì)列研究。Meta分析結(jié)果顯示,染發(fā)劑使用人群發(fā)生非霍奇金淋巴瘤的風(fēng)險(xiǎn)增加14%(OR=1.14 95%CI 1.01-1.29)。在病例對(duì)照研究OR值1.12(95%CI 0.99-1.26)及隊(duì)列研究中1.16(95%CI 0.91-1.69)兩者并無(wú)太大差異。亞組分析中女性使用者發(fā)生NHL的風(fēng)險(xiǎn)較男性增加;染發(fā)劑使用強(qiáng)度上,使用20年以上群體1.34(95%CI 1.04-1.92);地區(qū)之間同樣存在差異。結(jié)論:染發(fā)劑使用增加NHL發(fā)病風(fēng)險(xiǎn),尤其是使用時(shí)間及女性群體中,另外使用次數(shù)較多或者使用年限較久的人群因注意盡量減少暴露于染發(fā)劑產(chǎn)品。不過(guò)仍需要針對(duì)NHL亞型的研究來(lái)進(jìn)一步評(píng)估染發(fā)劑使用的危害。
[Abstract]:Objective: to evaluate and explore the relationship between hair dye use and the risk of non-Hodgkin 's lymphoma in order to enrich the knowledge of etiology and epidemiology of non-Hodgkin 's lymphoma. To provide reference for screening and protection of susceptible populations and for the prevention of clinical epidemic. Methods: Chinese "non-Hodgkin 's lymphoma", "lymphoma", "hair dye". "Case-control and / or cohort study", "risk factors". "non-Hodgkin lymphoma OR Lymphoma OR NHLLAND' Hair dye OR hair colorants OR hair coloring OR hair coloring agent OR. Hair color / colour). "AND(risk OR OR OR case-control study" and synonyms are keywords. The full text databases of Chinese academic journals, Wanfang full-text database, PubMedthe Cochrane Library and Embase database were searched respectively. At the same time, additional retrieval and manual retrieval were carried out to collect published case-control studies. According to the NOS scale, the quality of the literature was comprehensively evaluated and relevant data were extracted. A case-control study and a cohort study were conducted to evaluate the relationship between hair dye use and NHL with OR value and 95% confidence interval. I2 was used for heterogeneity in the literature. The data were analyzed by RevMan 5.2 software and Stata 12.0 software. Results: 16 articles were included. Among them, 13 case-control studies and 3 cohort studies. Meta-analysis results showed. The risk of non-Hodgkin 's lymphoma was increased by 1.14 95 CI 1.01-1.29 and OR 1.12 in the case-control study. 95 CI 0.99-1.26) and in the cohort study 1.16 / 95 CI 0.91-1.69). There was no significant difference between the two groups. In subgroup analysis, the risk of NHL among female users was higher than that of men. The intensity of hair dyeing agent was 1.3495 CI 1.04-1.92 in the group of more than 20 years; Conclusion: hair dye use increases the risk of NHL, especially in women and women. In addition, people who use more frequently or for a longer period of time take care to minimize exposure to hair dye products. However, a study of NHL subtypes is needed to further assess the harm of hair dye use.
