【摘要】：目的：通過檢測(cè)NSCLC患者中血清MMP-9、TLR4的表達(dá)水平,探討血清TLR4、MMP-9在NSCLC轉(zhuǎn)移過程中的相關(guān)性,分析它們?cè)谠\斷效能及及臨床預(yù)后判斷價(jià)值。方法：選自2015年9月1日至2016年1月1日就診于南昌大學(xué)第一附屬醫(yī)院呼吸內(nèi)科的34例非小細(xì)胞肺癌患者為實(shí)驗(yàn)組(NSCLC組),同一時(shí)期的門診健康體檢者15例為正常對(duì)照組,用酶聯(lián)免疫分析方法(ELISA)分別檢測(cè)各血清中MMP-9、TLR4的表達(dá)水平。結(jié)果：1、NSCLC組MMP-9、TLR4的表達(dá)水平均較正常對(duì)照組高,具有統(tǒng)計(jì)學(xué)意義(P0.05)。2、NSCLC伴遠(yuǎn)處轉(zhuǎn)移的患者血清MMP-9、TLR4高于無遠(yuǎn)處轉(zhuǎn)移的NSCLC患者；晚期(III/IV) NSCLC患者血清MMP-9、TLR4高于早期(Ⅰ/Ⅱ)NSCLC患者,統(tǒng)計(jì)學(xué)分析示均有P0.05。3、線性回歸分析NSCLC患者血清中MMP-9、TLR4的相關(guān)性,結(jié)果示R=0.939,P0.001,有統(tǒng)計(jì)學(xué)意義。4、血清MMP-9、TLR4在診斷NSCLC的ROC的AUC分別為0.776、0.702,兩者的靈敏度和特異度分別為67.56%和80.00%、80%和50%。5、NSCLC患者血清中MMP-9、TLR4的表達(dá)水平與遠(yuǎn)處轉(zhuǎn)移及臨床TNM分期呈相關(guān)性,均有P0.05,與年齡、性別、吸煙、病理類型等無明顯相關(guān)性,統(tǒng)計(jì)學(xué)分析示(P0.05)。結(jié)論：1、NSCLC患者與正常人群相比,其血清MMP-9、TLR4的表達(dá)水平顯著升高。2、在NSCLC中,伴遠(yuǎn)處轉(zhuǎn)移的患者與無遠(yuǎn)處轉(zhuǎn)移的相比,血清MMP-9、 TLR4顯著升高；晚期(III/IV)與早期(Ⅰ/Ⅱ)相比,兩血清指標(biāo)顯著升高。3、NSCLC患者血清MMP-9、TLR4表達(dá)水平呈顯著正相關(guān)性。4、血清MMP-9、TLR4可在一定程度上作為診斷NSCLC及判斷疾病預(yù)后的生化指標(biāo)。
[Abstract]:Objective: to investigate the correlation between serum MMP-9 and MMP-9 in the process of NSCLC metastasis by detecting the expression of MMP-9 in serum of patients with NSCLC. Methods: to analyze their diagnostic efficacy and clinical prognostic value. From September 1st 2015 to January 1st 2016, 34 patients with non-small cell lung cancer (NSCLC) in Department of Respiratory Medicine, first affiliated Hospital of Nanchang University were selected as experimental group (. NSCLC group. The expression of MMP-9 and TLR4 in serum was detected by Elisa in 15 healthy persons in the same period as the normal control group. Results: the expression of MMP-9 and TLR4 in each serum was detected by Elisa. The expression of MMP-9 and TLR4 in NSCLC group was significantly higher than that in normal control group, and had statistical significance in serum MMP-9 of patients with distant metastasis. TLR4 was higher in NSCLC patients without distant metastasis. The level of MMP-9 / TLR4 in patients with advanced stage II / IV NSCLC was higher than that in early stage (P 0.05.3). Linear regression analysis showed the correlation between MMP-9 and TLR4 in serum of patients with NSCLC. The results showed that the serum levels of MMP-9 and TLR4 in patients with NSCLC were significantly higher than those in control group (P 0.001). The AUC of TLR4 in the diagnosis of ROC in NSCLC was 0.776 and 0.702, and the sensitivity and specificity of the two were 67.56% and 80.00%, respectively. The expression of MMP-9 / TLR4 in serum of 80% and 50% patients with NSCLC was correlated with distant metastasis and clinical TNM stage (P0.05, age and sex). There was no significant correlation between smoking and pathological types. Statistical analysis showed that there was no significant correlation between the two groups. Conclusion compared with the normal controls, the serum MMP-9 of the patients with 1 / 1 NSCLC is higher than that of the normal controls. The expression of TLR4 was significantly increased in NSCLC patients with distant metastasis compared with those without distant metastasis.
