FHIT、RASSF1A蛋白在食管鱗狀細(xì)胞癌組織中的表達(dá)及臨床意義研究

【摘要】:目的:探討FHIT、RASSF1A蛋白在食管鱗狀細(xì)胞癌組織中的表達(dá)情況及與各臨床病理指標(biāo)之間的相互關(guān)系。方法:應(yīng)用免疫組織化學(xué)過氧化物酶標(biāo)記的鏈霉卵白素(Streptavidin-peroxidase, SP)的方法檢測抑癌基因FHIT和RASSF1A蛋白在8例慢性食管炎、18例低級別上皮內(nèi)瘤變、11例高級別上皮內(nèi)瘤變及39例食管鱗狀細(xì)胞癌組織中的表達(dá)情況。結(jié)果:FHIT在慢性食管炎、低級別上皮內(nèi)瘤變、高級別上皮內(nèi)瘤變及食管鱗狀細(xì)胞癌組織中的陽性率分別為12.50%(1/8)、33.33%(6/18)、36.3696(4/11)、43.59%(17/39),各個組之間的差別沒有統(tǒng)計學(xué)意義(P0.05)。RASSF1A在慢性食管炎、低級別上皮內(nèi)瘤變、高級別上皮內(nèi)瘤變及食管鱗狀細(xì)胞癌組織中的陽性表達(dá)率分別是75.00%(6/8)、50.00%(9/18)、45.45%(5/11)、33.33%(13/39),只有食管鱗癌組與慢性食管炎組比較差別有統(tǒng)計學(xué)意義(P=0.03)。FHIT在食管鱗狀細(xì)胞癌中的表達(dá)情況與TNM分期及淋巴轉(zhuǎn)移顯著相關(guān),其在I+II期表達(dá)的陽性率明顯的高于III期(P=0.00),在合并淋巴結(jié)轉(zhuǎn)移組的陽性表達(dá)比例顯著低于沒有淋巴結(jié)轉(zhuǎn)移組(P=0.01),而與年齡、性別、腫瘤的大小、腫瘤的生長部位、組織病理類型、浸潤層次、組織分化程度等無關(guān)(P0.05)。RASSF1A在食管鱗狀細(xì)胞癌中的表達(dá)水平與TNM分期及浸潤層次密切相關(guān),其在I+II期的陽性表達(dá)比例顯著的高于III期(P=0.02),在沒有侵犯到外膜層組的陽性率明顯的高于侵犯外膜層組(P=0.02),與年齡、性別、腫瘤的大小、腫瘤的生長部位、組織病理類型、淋巴轉(zhuǎn)移、組織分化程度等沒有關(guān)系(P0.05)。FHIT及RASSF1A蛋白在食管鱗狀細(xì)胞癌中的表達(dá)沒有明顯的相關(guān)性(r=-0.05,P=0.68)。結(jié)論:FHIT、RASSF1A蛋白的表達(dá)均與食管鱗癌的發(fā)生發(fā)展密切相關(guān)。FHIT蛋白可能在食管鱗癌的發(fā)生發(fā)展及侵襲轉(zhuǎn)移中發(fā)揮作用,而RASSF1A蛋白的檢測在食管鱗癌的臨床進展及預(yù)后判斷方面具有一定意義。
[Abstract]:Objective: to study FHIT. Expression of RASSF1A protein in esophageal squamous cell carcinoma and its relationship with clinicopathological parameters. Methods: streptavidin (SP) labeled by immunohistochemical peroxidase (PPO) was used to detect the expression of RASSF1A protein in esophageal squamous cell carcinoma (esophageal squamous cell carcinoma). Streptavidin-peroxidase. The tumor suppressor gene FHIT and RASSF1A protein were detected in 8 cases of chronic esophagitis and 18 cases of low grade intraepithelial neoplasia. 11 cases of high grade intraepithelial neoplasia and 39 cases of esophageal squamous cell carcinoma. Results: FHIT was detected in chronic esophagitis and low grade intraepithelial neoplasia. The positive rates of high grade intraepithelial neoplasia and esophageal squamous cell carcinoma were 12.50 / 8 and 33.336 / 18 / 36.3696 / 4 / 11, respectively. There was no statistically significant difference between the two groups in chronic esophagitis, low grade intraepithelial neoplasia. The positive expression rates in high-grade intraepithelial neoplasia and esophageal squamous cell carcinoma were 75.00 and 6- / 80.000, respectively. 33.33 / 13 / 39). Only the difference between esophageal squamous cell carcinoma group and chronic esophagitis group was statistically significant. The expression of P0.03FHIT in esophageal squamous cell carcinoma was significantly correlated with TNM stage and lymphatic metastasis. The positive rate of expression in III phase was significantly higher than that in III stage, and the positive rate in the group with lymph node metastasis was significantly lower than that in the group without lymph node metastasis (P < 0.01). But with age, sex, tumor size, tumor growth site, histopathological type, infiltration level. The expression level of P0.05U. RASSF1A in esophageal squamous cell carcinoma was closely related to TNM stage and invasive level. The percentage of positive expression in III phase was significantly higher than that in III stage, and the positive rate in the group without invasion of the outer layer was significantly higher than that in the group with invasion of the outer layer, and the positive rate was significantly higher than that in the group without invasion of the outer layer. Sex, tumor size, tumor growth site, histopathological type, lymphatic metastasis. There was no significant correlation between the expression of P0.05FHIT and RASSF1A protein in esophageal squamous cell carcinoma. Conclusion: FHIT. The expression of RASSF1A protein is closely related to the occurrence and development of esophageal squamous cell carcinoma. FHIT protein may play a role in the occurrence, development, invasion and metastasis of esophageal squamous cell carcinoma. The detection of RASSF1A protein has a certain significance in the clinical progress and prognosis of esophageal squamous cell carcinoma.
