【摘要】：目的:觀察中、西醫(yī)不同綜合康復(fù)方案治療急性腦出血(ACH)后對(duì)神經(jīng)內(nèi)分泌免疫網(wǎng)絡(luò)(NEIN)相關(guān)指標(biāo)的影響。方法:采用多中心、隨機(jī)對(duì)照試驗(yàn)的設(shè)計(jì)方法,258例患者分別用中醫(yī)綜合康復(fù)方案(中康組125例)和西醫(yī)綜合康復(fù)方案(西康組133例)治療,并觀察下丘腦-垂體-腎上腺軸(HPA)激素[促腎上腺皮質(zhì)激素釋放激素(CRH)、促腎上腺皮質(zhì)激素(ACTH)、皮質(zhì)醇(CS)]與免疫細(xì)胞因子[腫瘤壞死因子-α(TNF-α)、白細(xì)胞介素-6(IL-6)、白細(xì)胞介素-1β(IL-1β)],T淋巴細(xì)胞亞群(CD4+、CD8+、CD4+/CD8+)等相關(guān)指標(biāo)。結(jié)果:ACI后CRH、TNF-α等指標(biāo)均明顯高于正常值,說(shuō)明ACH時(shí)存在NEIN功能紊亂。治療28天后中康組在降低CRH、ACTH、CS和TNF-α、IL-6、IL-1β水平,改善CD4+、CD4+/CD8+含量等方面均優(yōu)于西康組(P0.05或0.01)。結(jié)論:中醫(yī)綜合康復(fù)治療較西醫(yī)綜合康復(fù)治療更能改善ACH后NEIN免疫學(xué)相關(guān)指標(biāo)。
[Abstract]:Aim: to observe the effect of different comprehensive rehabilitation regimen of western medicine on neuroendocrine immune network (NEINN) related indexes after treatment of acute cerebral hemorrhage (ACH). Methods: a multicenter, randomized controlled trial was used to treat 258 patients with comprehensive rehabilitation regimen of TCM (125 cases in Zhongkang group) and 133 cases in western medicine group (group Xikang), respectively. The relationship between hypothalamus-pituitary-adrenal axon (HPA) hormone and immune cytokines (tumor necrosis factor- 偽 -TNF- 偽, interleukin-6 (IL-6), interleukin (IL-6) was observed. The T lymphocyte subsets, CD 4, CD 8, CD 4 / CD 8) and so on. Results the indexes of NEIN TNF- 偽 after ACH were significantly higher than the normal value, which indicated that there was NEIN dysfunction in ACH. After 28 days of treatment, Zhongkang group was superior to Xikang group in decreasing the levels of CRH- ACTHN CS and TNF- 偽 IL-6 / IL-1 尾 and improving the content of CD4 / CD8 in CD4 group (P0.05 or 0.01). Conclusion: comprehensive rehabilitation therapy in traditional Chinese medicine can improve the immunological indexes of NEIN after ACH more than that in western medicine.
