【摘要】：目的:對缺血性中風急性期中西醫(yī)結(jié)合綜合治療方案在臨床中的應用進行隨機對照臨床試驗,觀察并評價其臨床療效。 方法:將所有入選病例隨機分為兩組,分別為中西醫(yī)結(jié)合綜合治療試驗組和西醫(yī)綜合治療對照組,對兩組病例治療情況進行觀測和記錄,用統(tǒng)計學方法處理相關(guān)數(shù)據(jù),進行療效評價和科學總結(jié)。 結(jié)果:治療后兩組的臨床癥狀及體征均得到了改善,但兩組的改善特點存在差異。兩組療效、神經(jīng)功能缺損(NIHSS)評分、中醫(yī)病類診斷標準評分、中醫(yī)證候量化積分、總的生活能力狀態(tài)(ADL)評分及生存質(zhì)量(QLI)指數(shù)評分比較在治療后7、14、28天后的改變存在明顯差異。兩組患者治療后NIHSS評分均有明顯改善,在治療后14天時試驗組優(yōu)于對照組(P0.05),28天時明顯優(yōu)于對照組(P0.01)。治療后28天時中醫(yī)病類診斷標準評分、中醫(yī)證候量化積分、ADL評分比較,試驗組明顯優(yōu)于對照組(P0.01)。試驗組在改善病人的生存質(zhì)量(QLI)指數(shù)方面明顯優(yōu)于對照組(P0.01)。治療后28天時試驗組基本恢復病例占31.43%,顯著進步者占65.71%,進步者占2.86%,對照組基本恢復者占7.35%,顯著進步者占22.06%,進步者占66.18%,稍進步者占4.41%,試驗組臨床總療效優(yōu)于對照組(P0.05)。兩組TC、TG、LDL-C、Fg均有降低,試驗組優(yōu)勢更加明顯(P0.01)。治療后對照組ALT明顯升高(P0.01),WBC明顯降低(P0.05)。 結(jié)論: 1.中西醫(yī)結(jié)合綜合治療方案合理應用能夠降低缺血性中風急性期患者死亡率,明顯促進患者的神經(jīng)功能恢復,改善患者的日常生活能力,提高患者遠期生存質(zhì)量,降低致殘率。2.能夠較好地控制血脂異常,降低纖維蛋白原,改善凝血功能。
[Abstract]:Objective : To observe and evaluate the clinical curative effect of integrated traditional Chinese medicine and western medicine in the treatment of ischemic stroke . Methods : All selected cases were randomly divided into two groups , the combination of western medicine and western medicine combination therapy group and western medicine comprehensive treatment control group , the treatment of two groups were observed and recorded , the correlation data was treated by statistical method , and the curative effect evaluation and scientific summary were carried out . Results : Compared with the control group ( P0.01 ) , the clinical symptoms and signs of the two groups were significantly better than those in the control group ( P0.01 ) . Conclusion : 1 . Reasonable application of integrated traditional Chinese medicine and western medicine can reduce the mortality of patients in acute stage of ischemic stroke , obviously promote the recovery of nerve function of patients , improve the daily life ability of the patients , improve the long - term survival quality of the patients and reduce the disability rate .
