摘要
目的:調(diào)查中國(guó)十省市第一代及第二代抗精神病藥治療精神分裂癥的使用現(xiàn)況,并初步分析其相關(guān)因素.方法:選擇2006年全國(guó)第2次精神藥物處方方式調(diào)查中,符合國(guó)際疾病和相關(guān)健康問(wèn)題分類第十版(ICD-10)精神分裂癥診斷標(biāo)準(zhǔn),并且接受單一、非長(zhǎng)效抗精神病藥治療的4232例患者為研究對(duì)象,收集一般資料、臨床癥狀及抗精神病藥物使用情況等數(shù)據(jù).結(jié)果:第二代抗精神病藥使用比例明顯高于第一代抗精神病藥(70.8％vs.29.2％,P＜0.01).使用頻率居前6位的藥物依次是利培酮(31.1％)、氯氮平(24.0％)、舒必利(10.3％)、氯丙嗪(9.7％)、奮乃靜(8.4％)和喹硫平(5.9％).與使用第二代藥物的患者相比,使用第一代藥物的患者年齡較大,男性比例較高,有家族史的比例較大,病程較長(zhǎng),家庭收入較低,就診次數(shù)較少,發(fā)作次數(shù)較多(均P ＜0.05);使用第二代藥物的患者中妄想幻覺(jué)狀態(tài)、言語(yǔ)行為紊亂、言語(yǔ)活動(dòng)增多及攻擊癥狀出現(xiàn)的比例高于使用第一代藥物的患者(均P ＜0.05),而陰性癥狀出現(xiàn)的比例較低(P＜0.01).Logistic回歸分析顯示,江西(OR=1.29)和四川(OR=1.08)、年齡大(OR=1.02)、發(fā)作次數(shù)多(OR=1.03)、從事農(nóng)業(yè)勞動(dòng)(OR=1.21)的患者更傾向于選擇第一代抗精神病藥(均P＜0.01).結(jié)論:第二代抗精神病藥在我國(guó)精神分裂癥患者的治療中占主導(dǎo)地位.
