通過(guò)血管鑄型技術(shù)及CT三維重建來(lái)探討子宮血管分布及血管網(wǎng)的意義

【摘要】:目的: 通過(guò)對(duì)家兔的血管鑄型技術(shù)、方法的進(jìn)一步探討及研究,從而獲得家兔子宮動(dòng)脈血管鑄型標(biāo)本,以及通過(guò)對(duì)人子宮血管行CT三維重建獲得的影像資料,共同來(lái)觀察子宮動(dòng)脈、子宮動(dòng)脈-卵巢支、子宮動(dòng)脈-輸卵管支的分布情況,以及對(duì)子宮動(dòng)脈血管網(wǎng)的研究和分布情況,進(jìn)一步對(duì)子宮動(dòng)脈血管網(wǎng)吻合支的研究。 方法: 家兔正常麻醉處死后,于腹主動(dòng)脈進(jìn)行插管的同時(shí)行髂外動(dòng)脈絲線結(jié)扎,來(lái)對(duì)家兔子宮及雙側(cè)輸卵管、卵巢行血管鑄型。鑄型材料采用乙酸乙酯和過(guò)氯乙烯及鄰苯二甲酸二丁酯、部分油畫染料充分?jǐn)噭蚝笞鰹樘畛鋭,在插管后再行補(bǔ)灌注、腐蝕、沖洗、標(biāo)本休整、標(biāo)本保存等步驟,來(lái)完成家兔子宮血管鑄型標(biāo)本的制備。人體子宮動(dòng)脈影像圖像可通過(guò)行靜脈血管打入血管造影劑,后經(jīng)CT三維重建獲得子宮動(dòng)脈血管圖像。 結(jié)果: 通過(guò)對(duì)家兔子宮鑄型標(biāo)本的觀察,可以清晰的觀察到子宮動(dòng)脈血管的分布情況,以及子宮血管的二、三級(jí)分支及分布情況,對(duì)于細(xì)小的血管網(wǎng)借助掃描電鏡可觀察到子宮動(dòng)脈卵巢支、輸卵管支的血管分布及血管網(wǎng)吻合支情況。主要表現(xiàn)為: 1通過(guò)血管鑄型獲得的子宮血管情況: (1)子宮動(dòng)脈血管網(wǎng)分布及吻合支分布情況: 1兔子宮動(dòng)脈于宮體兩側(cè)迂曲向上,于宮體部發(fā)出子宮動(dòng)脈螺旋支,雖較為細(xì)小,,但可通過(guò)放大鏡或者掃描電鏡觀察到子宮體部存在明顯的交通支,且分布十分密集,進(jìn)一步證明了子宮動(dòng)脈存在明顯的相互供血傾向。2子宮動(dòng)脈上行支為子宮體、卵巢和輸卵管供血,下行支由于取標(biāo)本原因未能顯示。3子宮動(dòng)脈上行支迂曲延輸卵管走形發(fā)出子宮動(dòng)脈卵巢支,可見(jiàn)子宮動(dòng)脈卵巢支與卵巢動(dòng)脈相互吻合情況。 2.通過(guò)對(duì)人靜脈血管打入造影劑,經(jīng)CT三維重建獲得的子宮動(dòng)脈血管圖像情況: (1)子宮動(dòng)脈血管分布情況: 1正常子宮動(dòng)脈主干于宮頸內(nèi)口水平依次分出膀胱支、輸尿管支、上行支和下行支等動(dòng)脈血管,可清晰看到子宮及其上行支,下行支、膀胱支及輸尿管支由于由于血管較細(xì)造影劑流入較少故未能顯示。2可觀察到較粗大的子宮動(dòng)脈上行支,沿子宮壁的側(cè)緣迂曲向上,沿途可見(jiàn)子宮動(dòng)脈上行支發(fā)出多條弓狀動(dòng)脈,弓狀動(dòng)脈再發(fā)出若干螺旋動(dòng)脈,分布于子宮前、后面,在到達(dá)子宮底水平后,在子宮角處發(fā)出多條分支,分別為宮底支、卵巢支和輸卵管支;子宮動(dòng)脈上行支主要為子宮體、卵巢和輸卵管供血。 (2)子宮動(dòng)脈血管網(wǎng)分布形及吻合支分布情況: 1子宮動(dòng)脈以中軸線為中心,可清晰看到兩側(cè)存在明顯的交通支,盡管較為微細(xì),但分布較為集中且密集,進(jìn)一步證明了子宮動(dòng)脈存在明顯的相互供血傾向。2可見(jiàn)沿子宮壁迂曲向上的子宮動(dòng)脈上行支于子宮角處直接發(fā)出較粗大的卵巢支,經(jīng)過(guò)卵巢固有韌帶向卵巢提供部分供血,由此可見(jiàn),卵巢的血供不單有卵巢動(dòng)脈供給,還有子宮動(dòng)脈卵巢支。 結(jié)論: 通過(guò)應(yīng)用血管鑄型技術(shù)及CT三維重建圖像處理后,都可清晰顯示子宮動(dòng)脈分支、分布及血管網(wǎng)吻合支情況,為婦產(chǎn)科疾病的臨床診斷、手術(shù)治療等方面提供血管形態(tài)學(xué)依據(jù),為指導(dǎo)子宮動(dòng)脈栓塞治療及新式婦產(chǎn)科手術(shù)的開(kāi)展提供解剖學(xué)平臺(tái)。
