【摘要】：目的:回顧性研究關(guān)節(jié)鏡下手術(shù)治療盤狀半月板的長期結(jié)果;從性別、癥狀持續(xù)時間、手術(shù)時年齡、盤狀半月板類型、半月板損傷類型等多個方面分析影響手術(shù)結(jié)果的因素。方法:回顧性分析2008~2013年期間于我院手術(shù)治療的97例106膝盤狀半月板。平均隨訪時間27.3±18.8個月。主要手術(shù)方式包括半月板修整成形術(shù)、修整成形+固定術(shù)和半月板全切術(shù)。記錄隨訪末期Lysholm評分、IKDC評分、Tegner運(yùn)動評分及VAS疼痛評分;手術(shù)結(jié)果按照Ikeuchi分級標(biāo)準(zhǔn)進(jìn)行分級。結(jié)果:所有病例隨訪期手術(shù)結(jié)果 Ikeuchi分級為:優(yōu)64膝(60.4%),良27膝(25.5%),可10膝(9.4%),差5膝(4.7%);總體優(yōu)良率達(dá)85.9%。手術(shù)時的年齡及術(shù)前癥狀持續(xù)時間對手術(shù)結(jié)果有影響,而患者性別、盤狀半月板類型、半月板損傷類型、手術(shù)方式均無明顯影響。結(jié)論:關(guān)節(jié)鏡下手術(shù)治療盤狀半月板,年輕患者及早期手術(shù)治療患者的療效較好,建議對盤狀半月板損傷應(yīng)盡早行關(guān)節(jié)鏡下手術(shù)治療。
[Abstract]:Aim: to study the long-term results of arthroscopic surgical treatment of discoid meniscus, and to analyze the factors influencing the outcome of the operation from the aspects of sex, duration of symptoms, age during operation, type of discoid meniscus and meniscal injury. Methods: 97 cases (106 knees) of discoid meniscus treated in our hospital from 2008 to 2013 were analyzed retrospectively. The mean follow-up time was 27.3 鹵18.8 months. The main surgical methods include meniscus repair, repair and fixation, and total meniscectomy. The Lysholm score, IKDC score, Tegner motor score and VAS pain score were recorded at the end of follow-up, and the surgical results were graded according to the Ikeuchi classification standard. Results: the Ikeuchi classification of all cases was excellent in 64 knees (60.4%), good in 27 knees (25.5%), fair in 10 knees (9.4%) and poor in 5 knees (4.7%), and the overall excellent and good rate was 85.9%. The age at the time of operation and the duration of symptoms before operation had an effect on the outcome of the operation, but the gender, type of discoid meniscus, type of meniscus injury and operation mode had no significant effect on the outcome of the operation. Conclusion: arthroscopic surgery in the treatment of discoid meniscus, young patients and early surgical treatment of patients is better. It is suggested that arthroscopic surgery should be performed as soon as possible for discoid meniscus injury.
