256層螺旋計算機斷層攝影術(shù)對右心耳及其毗鄰形態(tài)結(jié)構(gòu)的定量研究

【摘要】:目的:采用256層螺旋計算機斷層攝影術(shù)(MSCT)對右心耳及其毗鄰的形態(tài)結(jié)構(gòu)進行定量研究,為右心耳臨床介入和外科治療提供影像學(xué)依據(jù)。方法:回顧性分析我院行計算機斷層攝影術(shù)血管造影(CTA)檢查結(jié)果陰性者200例,男96例,女104例,40歲者29例,40~60歲者114例,60歲者57例。將原始掃描數(shù)據(jù)進行右心耳重建,對右心耳容積及徑線等參數(shù)進行測量,并從性別和年齡方面進行比較,提出右心耳各參數(shù)95%正常參考值范圍。結(jié)果:(1)右心耳容積、高度、基底部長徑、基底部周長、正位展距男性均大于女性,差異均有統(tǒng)計學(xué)意義(P均0.05)。經(jīng)體表面積標化后,右心耳基底部長短徑、面積及周長女性均大于男性,差異均有統(tǒng)計學(xué)意義(P均0.05)。(2)40歲者右心耳正位展角及正位展距較40~60歲及60歲者大(P0.05),40~60歲者與60歲者間差異無統(tǒng)計學(xué)意義(P0.05)。(3)提出右心耳各參數(shù)95%正常參考值范圍:容積4.83~19.97 ml、高度21.46~41.80 mm、基底部長徑25.89~41.65 mm、基底部短徑19.34~36.24 mm、基底部面積459.30~1 093.28 mm~2、基底部周長86.16~147.04 mm、正位展角1.67°~31.23°、正位展距8.07~34.37 mm。結(jié)論:256層MSCT可以對右心耳解剖結(jié)構(gòu)相關(guān)參數(shù)進行定量研究,并初步建立右心耳各參數(shù)95%正常參考值范圍,為臨床治療右心耳相關(guān)疾病提供影像學(xué)參考。
[Abstract]:Objective: to quantitatively study the morphologic structure of the right atrial appendage and its adjacent by 256-slice spiral computed tomography (MSCT), and to provide the imaging basis for the clinical intervention and surgical treatment of the right atrial appendage. Methods: a retrospective analysis was made on 200 cases with negative (CTA) results of computed tomography angiography in our hospital, including 96 males, 104 females, 29 40 years old, 114 40 years old and 57 60 years old. The original scanning data were reconstructed and the parameters such as volume and diameter of the right atrial appendage were measured and compared from gender and age. The range of 95% normal reference values of each parameter of the right atrial appendage was put forward. Results: (1) the volume and height of the right atrial appendage, the length of the base, the circumference of the base and the distance between the male and the female were significantly higher than those in the female (all P 0.05). After the surface area was standardized, the length, area and circumference of the base of the right atrial appendage were larger in the female than in the male. There was significant difference between the two groups (P < 0. 05). (2). The angle and distance of right atrial appendage in 40 years old were larger than those in 40 and 60 years old (P0.05). There was no significant difference between 4060 years old and 60 years old (P0.05). (3). The range of 95% normal reference value of each parameter of right atrial appendage was put forward: volume 4.83 ~ 19.97 ml, height 21.46 ~ 41.80 mm, base length and diameter 25.89 ~ 41.65 mm,. Conclusion: 256-layer MSCT can be used to quantitatively study the anatomical structure parameters of right atrial appendage, and to establish 95% normal reference range of right atrial appendage parameters, which can provide imaging reference for clinical treatment of right atrial appendage related diseases.
