【摘要】：目的探討肝切除聯(lián)合術中熱消融治療肝硬化背景下多灶性肝癌的有效性和安全性。方法回顧性分析63例接受肝切除聯(lián)合術中熱消融治療肝硬化背景下多灶性肝癌患者的臨床資料,分析患者的治療效果及并發(fā)癥。結果 63例患者共切除病灶85個,病灶平均最大直徑(4.66±3.82)cm,熱消融治療病灶146個,病灶平均最大直徑(1.48±0.66)cm。146個病灶完全消融率達100%(146/146),局部復發(fā)率4.11%(6/146)�；颊�1、2、3、4年無瘤生存率分別為54.82%、31.28%、12.03%、6.02%;1、3、5年累積生存率分別為83.51%、56.83%、45.20%。未出現(xiàn)治療相關的死亡病例。肝切除相關嚴重并發(fā)癥8例(8/63,12.70%)。未出現(xiàn)熱消融相關嚴重并發(fā)癥。結論肝切除聯(lián)合術中熱消融治療肝硬化背景下多灶性肝癌安全、有效,擴大了肝切除術的適應證。
[Abstract]:Objective to evaluate the efficacy and safety of hepatectomy combined with intraoperative thermal ablation in the treatment of multifocal liver cancer in the context of cirrhosis. Methods the clinical data of 63 patients with multifocal liver cancer treated with hepatectomy combined with intraoperative thermal ablation were analyzed retrospectively. The therapeutic effects and complications of the patients were analyzed. Results Eighty-five lesions were resected in 63 patients. The mean maximum diameter of the lesions was (4.66 鹵3.82) cm, thermal ablation for 146 lesions. The complete ablation rate of the lesions was 100% (146 / 146) with the mean maximum diameter of (1.48 鹵0.66) cm.146. The local recurrence rate was 4.11% (6 / 146). The tumor-free survival rates were 54.82%, 31.28% and 12.03%, respectively, and the 5-year cumulative survival rates were 83.51%, 56.83% and 45.20%, respectively. There were no treatment-related deaths. Severe complications associated with hepatectomy were found in 8 cases (8 / 63, 12.70%). There were no serious complications associated with thermal ablation. Conclusion hepatectomy combined with intraoperative thermal ablation is safe and effective in the treatment of multifocal liver cancer in the background of liver cirrhosis, which expands the indication of hepatectomy.
