雙低掃描技術(shù)結(jié)合雙源CT Flash Spiral模式在冠狀動脈成像中的應(yīng)用

【摘要】:目的探討雙源CT Flash Spiral掃描模式下,270碘對比劑結(jié)合低管電壓及正弦確認(rèn)迭代重組(SAFIRE)技術(shù)在改善冠狀動脈成像圖像質(zhì)量及降低輻射劑量中的價(jià)值。方法將平均心率65次/分的82例疑診冠心病患者隨機(jī)分為A、B兩組,A組42例,采用管電壓120 k V、350 mg I/m L的對比劑,重組圖像采用濾過反投影(FBP)法;B組40例,采用管電壓80 k V或100 k V、270 mg I/m L的對比劑,重組圖像同時(shí)采用FBP法(B1組)和SAFIRE法(B2組)。A、B兩組均采用Flash Spiral模式掃描,采集圖像時(shí)間為R-R間期60%。記錄劑量長度乘積(DLP),計(jì)算有效輻射劑量(ED)。測量A組與B組兩種重組法主動脈根部、左冠狀動脈及右冠狀動脈起始部血管內(nèi)CT值和圖像噪聲(SD),計(jì)算主觀圖像質(zhì)量評分、圖像噪聲(SD)、信噪比(SNR)、對比噪聲比(CNR)。結(jié)果 A、B兩組的ED分別為(1.18±0.21)m Sv、(0.55±0.19)m Sv,差異有統(tǒng)計(jì)學(xué)意義(t=14.22,P0.01);A組與B組兩種重組法B1、B2冠狀動脈圖像質(zhì)量評分分別為1.57±0.60、1.85±0.59、1.55±0.60,SD分別為(17.62±2.44)HU、(27.25±8.06)HU、(19.21±6.21)HU,B1與B2組間均有顯著統(tǒng)計(jì)學(xué)差異(P0.01),A與B2組間均無統(tǒng)計(jì)學(xué)差異(P0.05);血管內(nèi)強(qiáng)化CT值、SNR、CNR B2組均高于A組,有統(tǒng)計(jì)學(xué)差異(P0.05)。結(jié)論雙源CT Flash Spiral掃描模式下,270碘對比劑結(jié)合低管電壓及SAFIRE技術(shù)能明顯降低輻射劑量,并能獲得較好的圖像質(zhì)量。
[Abstract]:Objective to investigate the value of double source CT Flash Spiral scan with low tube voltage and sinusoidal confirmation iterative recombination (SAFIRE) in improving the image quality of coronary artery imaging and reducing the radiation dose. Methods Eighty-two patients with suspected coronary heart disease with an average heart rate of 65 beats / min were randomly divided into two groups: group A (n = 42) and group B (n = 40). The tube voltage of 80 kV or 100 kV of 270 mg I / mL was used. The reconstructed images were scanned by Flash Spiral mode in both groups of FBP (B1 group) and SAFIRE (B2 group). The dose length product (DLP) was recorded and the effective radiation dose (Ed) was calculated. Intravascular CT value and image noise (SD) of aortic root, left coronary artery and right coronary artery in group A and group B were measured, and subjective image quality scores, image noise (SD), signal-to-noise ratio (SNR) and contrast noise ratio (CNR) were calculated. Results Ed was (1.18 鹵0.21) mSvand (0.55 鹵0.19) mSvin in group A and group B, respectively. There was significant difference between group A and group B in the image quality of coronary artery between group A and group B respectively (1.57 鹵0.601.85 鹵0.591.55 鹵0.60 SD = (17.62 鹵2.44) HUU, (27.25 鹵8.06) HUU, (19.21 鹵6.21) HUB1 and B2 groups, respectively. There was no statistical difference (P0.05), the value of enhanced intravascular CT was significantly higher in the CNR B2 group than in the A group. There was statistical difference (P0.05). Conclusion under the dual-source CT Flash Spiral scan mode, the low tube voltage and SAFIRE can significantly reduce the radiation dose and obtain better image quality with the combination of low-tube voltage and SAFIRE.
