【摘要】：目的測算社區(qū)兒童健康管理服務成本,為社區(qū)兒童健康管理服務經(jīng)費預算及管理提供科學依據(jù)。方法通過文獻研究及專題討論建立備選指標庫;經(jīng)兩輪德爾菲法專家咨詢,確定兒童健康管理服務標準項目內容及工作步驟;隨機抽取廣東省深圳市60家社康中心進行現(xiàn)場調查,測定兒童健康管理服務工作時間及2013年服務量,并預測2015年兒童健康管理成本。結果社區(qū)兒童健康管理項目包括三大類、15項服務內容和58條工作步驟;60家社康中心2013年兒童健康管理服務調整工作時間合計78 646.9 h;按調查社康、全市社康、基層專業(yè)公共衛(wèi)生機構當年在編人員平均收入水平和專家建議4種人力成本價格得出兒童健康管理服務總成本分別為651.8、659.7、738.3和846.1萬元;人均成本分別為6.2、6.2、7.0和8.0元/人年;規(guī)范管理一名0~3歲兒童的單位成本分別為134.1、135.7、151.9和174.1元/人年,兒童健康管理服務應占基本公共衛(wèi)生服務經(jīng)費的9.2%,預測2015年深圳市社區(qū)兒童健康管理服務若需完全達到國家規(guī)范標準,需要人均成本分別為10.4、10.6、11.8和13.5元/人年;深圳市全市投入兒童健康管理經(jīng)費約需1億元。結論 2013年深圳市兒童健康管理服務人均成本為6.2元/人,高于目前政府財政對兒童健康管理服務的實際投入標準,建議2015年增至人均10.0元/年。
[Abstract]:Objective to measure the cost of community child health management service, and to provide scientific basis for the budget and management of community child health management service. Methods the alternative index database was established through literature research and special discussion, and the contents and working steps of the standard items of child health management service were determined through two rounds of Delfie expert consultation. 60 social health centers in Shenzhen City, Guangdong Province, were randomly selected for field investigation to determine the working time of child health management service and the service volume in 2013, and to predict the cost of child health management in 2015. Results the community child health management project included three categories, 15 service contents and 58 working steps, and the total working time of child health management service in 60 social health centers was 78 646.9 hours in 2013. According to the average income level of the staff and the four kinds of human cost prices recommended by experts, the total cost of child health management service was 651.8659.7738.3 and 8.461 million yuan, respectively, and the per capita cost was 6.2, 6.2, 7.0 and 8.0yuan per person-year, respectively, according to the survey of social health, social health of the city and the average income level of staff in the same year and the four kinds of human cost prices suggested by experts. The unit cost of standardized management of a child aged 0 to 3 years is 134.1135.7151.9 and 174.1 yuan / person year, respectively. The child health management service should account for 9.2% of the basic public health service expenditure. It is predicted that if the community child health management service in Shenzhen in 2015 needs to fully meet the national standard, the per capita cost will be 10.4, 10.6, 11.8 and 13.5 yuan per person year, respectively. Shenzhen City invested about 100 million yuan in child health management. Conclusion in 2013, the per capita cost of child health management service in Shenzhen was 6.2 yuan per person, which was higher than the actual investment standard of government finance in child health management service at present, and it was suggested that it should be increased to 10.0 yuan per person in 2015.
