老年人步態(tài)移動(dòng)穩(wěn)定性的增齡特征研究

【摘要】:目的:探討老年人步態(tài)移動(dòng)穩(wěn)定性的增齡特征,為老年人跌倒的預(yù)防提供理論指導(dǎo)。方法:采用美國(guó)APDM Mobility LabTM可穿戴式步態(tài)平衡測(cè)試系統(tǒng)對(duì)58名不同年齡段(60~90歲)的老年人從行走步態(tài)、轉(zhuǎn)身動(dòng)作和坐-立動(dòng)作三個(gè)維度進(jìn)行了i TUG(Instrumented Timed Up and Go,改良坐立行走計(jì)時(shí))測(cè)試。結(jié)果:(1)TUG總時(shí)間與i TUG總時(shí)間:≥80歲組與60~70歲組具有顯著差異(P0.05),70~80歲組和60~70歲組具有顯著差異(P0.05),而≥80歲組與70~80歲組無(wú)顯著性差異(P0.05)。且測(cè)試結(jié)果均表現(xiàn)出隨著年齡增加總時(shí)間逐漸增加。(2)行走步態(tài)參數(shù):步長(zhǎng)、步速及下肢小腿環(huán)節(jié)活動(dòng)范圍等移動(dòng)能力相關(guān)指標(biāo),三組間都存在顯著性差異(P0.05),均表現(xiàn)出隨年齡增加呈逐級(jí)遞減趨勢(shì)。而軀干相關(guān)的運(yùn)動(dòng)參數(shù)3個(gè)年齡組之間無(wú)顯著性差異(P0.05)。(3)轉(zhuǎn)身動(dòng)作相關(guān)參數(shù):≥80歲組與70~80歲組均不具有顯著性差異(P0.05),而與60~70歲組之間存在顯著性差異(P0.05)。(4)坐-立動(dòng)作參數(shù),坐-立時(shí)間和坐-立軀干活動(dòng)范圍(角度)3組間均無(wú)顯著性差異(P0.05)。而在轉(zhuǎn)身坐下的峰值角速度和時(shí)間上,≥80歲齡組則與70~80歲組和60~70歲組均存在顯著性差異(P0.05)。軀干活動(dòng)范圍(角度),60~70歲組與其它兩組均存在顯著性差異(P0.05)。且隨年齡增加,轉(zhuǎn)身坐下軀干活動(dòng)范圍(角度)呈減小趨勢(shì)。結(jié)論:老年人步態(tài)移動(dòng)穩(wěn)定性的增齡特征明顯,普遍存在隨年齡增加步態(tài)移動(dòng)能力下降趨勢(shì),尤其在縱向直線移動(dòng)控制能力方面,表現(xiàn)出明顯的年齡相關(guān)性,而在側(cè)向移動(dòng)控制能力方面,70歲以后年齡相關(guān)性減弱,提示70歲可能是高跌倒風(fēng)險(xiǎn)的臨界年齡。
