【摘要】：2012年10月22日,新鄉(xiāng)市衛(wèi)生局衛(wèi)生監(jiān)督員依法對(duì)新鄉(xiāng)市某醫(yī)療機(jī)構(gòu)進(jìn)行現(xiàn)場(chǎng)監(jiān)督檢查時(shí)發(fā)現(xiàn):1該醫(yī)療機(jī)構(gòu)現(xiàn)場(chǎng)不能提供《放射診療許可證》從事放射診療工作;2放射工作人員李某現(xiàn)場(chǎng)不能提供《放射工作人員證》從事放射診療工作,上崗時(shí)未佩戴個(gè)人劑量計(jì);3放射工作場(chǎng)所未配備受檢人員防護(hù)用品;4現(xiàn)場(chǎng)未能提供該醫(yī)療機(jī)構(gòu)X射線機(jī)房職業(yè)病危害放射防護(hù)預(yù)評(píng)價(jià)報(bào)告;5該醫(yī)療機(jī)構(gòu)X射線機(jī)已投入臨床使用,現(xiàn)場(chǎng)不能提供職業(yè)病危害控制效果評(píng)價(jià)報(bào)告及竣工驗(yàn)收合格證明文件;6現(xiàn)場(chǎng)發(fā)現(xiàn)X射線機(jī)使用記錄《門(mén)診工作日志》一本,使用日期:2012年9月15日至2012年10月22日;7 X射線機(jī)房入口處未設(shè)置電離輻射警告標(biāo)志,工作指示燈不能正常使用。警告期限到期后,新鄉(xiāng)市衛(wèi)生局衛(wèi)生監(jiān)督員于2012年11月26日再次對(duì)該醫(yī)療機(jī)構(gòu)進(jìn)行監(jiān)督檢查,發(fā)現(xiàn)以上違法事實(shí)仍然存在。本案2012年11月26日受理,2012年11月30日立案[1]。
[Abstract]:On October 22, 2012, when the health supervisor of Xinxiang City Health Bureau carried out on-site supervision and inspection of a medical institution in Xinxiang City according to law, it was found that: (1) the medical institution could not provide the "Radiological diagnosis and treatment permit" on the spot to engage in the work of radiology diagnosis and treatment; (2) radiation worker Li Mou can't provide "radiation worker's card" to be engaged in radiology diagnosis and treatment work, without personal dosimeter on duty; 3 radiation work place is not equipped with protective equipment for the person under examination; (4) the pre-evaluation report of the occupational hazards of the X-ray machine room of the medical institution could not be provided on the spot; (5) the X-ray machine of the medical institution has been put into clinical use, and the evaluation report of occupational disease hazard control effect and the certificate of acceptance and acceptance can not be provided on the spot; (6) A copy of the working log of outpatient service of X-ray machine was found on the spot, which was used from September 15, 2012 to October 22, 2012. 7. There was no ionizing radiation warning sign at the entrance of the X-ray machine room, and the working indicator lamp could not be used normally. After the expiration of the warning period, the health supervisor of Xinxiang City Health Bureau again inspected the medical institution on November 26, 2012, and found that the above illegal facts still exist. The case was admitted on 26 November 2012 and opened on 30 November 2012 [1].
