【摘要】：目的探討絕經(jīng)女性中心型肥胖與血脂異常的關(guān)系。方法依據(jù)蘇州工業(yè)園區(qū)2014年慢性病社區(qū)綜合防治項(xiàng)目,隨機(jī)抽取某社區(qū)2 000名居民,選取其中健康自然絕經(jīng)12個(gè)月以上的女性323人,測(cè)量身高、體重、腰圍、血壓,采血測(cè)甘油三酯(TG)、總膽固醇(TC)、高密度脂蛋白膽固醇(HDL-C)、低密度脂蛋白膽固醇(LDLC)水平。根據(jù)衛(wèi)生行業(yè)標(biāo)準(zhǔn)《成人體重判定》,腰圍≥85 cm診斷為中心型肥胖,根據(jù)《中國成人血脂異常防治指南(2007)》中TG、TC、HDL-C和LDL-C的水平診斷血脂異常。采用多因素Logistic回歸分析中心型肥胖與血脂異常發(fā)病風(fēng)險(xiǎn)之間的關(guān)系。結(jié)果該絕經(jīng)女性人群中心型肥胖占13.6%(44/323),血脂異常檢出率為29.7%(96/323),中心型肥胖組血脂異常檢出率顯著高于正常組(43.2%vs 27.6%,P=0.036)。腰圍水平與TG及LDL-C呈正相關(guān)(TG:r=0.28,P0.01;LDL-C:r=0.20,P0.01),與HDL-C呈負(fù)相關(guān)(r=-0.26,P0.01)。多因素Logistic回歸分析顯示中心型肥胖組患血脂異常的OR值為2.07(95%CI 1.04~4.13,P=0.039),其中中心型肥胖組TG升高和HDL-C降低的OR值分別為3.81(95%CI1.69~8.60,P=0.001)和3.19(95%CI 1.36~7.52,P=0.008)。結(jié)論中心型肥胖是絕經(jīng)女性血脂異常的危險(xiǎn)因素。
