鐵過載人群臨床血液學(xué)特征及對健康影響分析

【摘要】:目的機體不同鐵負(fù)荷水平可引起血液學(xué)特征的變化,對機體帶來不良影響。本研究旨在分析成年人群鐵過載狀態(tài)下臨床血液學(xué)特征的變化及對部分健康因素的影響,為深入探討鐵過載對人群健康的影響及疾病預(yù)防提供參考。方法選取2014年1月~12月某醫(yī)院的體檢人群,按照血清鐵蛋白(serum ferritin)水平隨機抽取鐵狀態(tài)正常者113名(男性:15~200μg/L;女性:15~150μg/L)和鐵過載者124名(男性:200μg/L;女性:150μg/L),年齡為40~70歲。采集空腹靜脈血,檢測機體鐵代謝有關(guān)的血液指標(biāo),如血紅蛋白、外周血細(xì)胞數(shù)量、血清鐵、轉(zhuǎn)鐵蛋白、肝功能相關(guān)指標(biāo)及礦物質(zhì)水平等。對比分析兩組之間的差異。結(jié)果鐵代謝相關(guān)血液學(xué)指標(biāo)結(jié)果顯示,鐵過載組人群血紅蛋白(Hb)平均水平為156.16g/L,高于鐵正常組147.76g/L(P0.001);紅細(xì)胞數(shù)量變化分析顯示,鐵過載組紅細(xì)胞計數(shù)達5.01×1012/L,明顯高于鐵正常組的4.83×1012/L(P0.05);其紅細(xì)胞壓積、平均紅細(xì)胞體積、平均紅細(xì)胞血紅蛋白含量等也均顯著高于鐵正常組(P均0.05)。鐵過載組人群血清鐵為22.93μmol/L,顯著高于鐵正常組17.83μmol/L(P0.05);鐵過載組人群轉(zhuǎn)鐵蛋白飽和度達到36%,顯著高于鐵正常組的28%(P0.05);不飽和鐵結(jié)合力和轉(zhuǎn)鐵蛋白濃度均顯著低于鐵正常組(P0.05);兩組間總鐵結(jié)合力無統(tǒng)計學(xué)差異(P0.05)。血清蛋白水平分析顯示,鐵過載組總蛋白濃度為76.02g/L,高于鐵正常組的73.64g/L(P0.05);白蛋白濃度為44.83g/L,顯著高于鐵正常組的43.55g/L(P0.05)。血清礦物質(zhì)水平分析顯示,鐵過載組血清鈣平均水平為2.42mmol/L,顯著高于鐵正常組的2.39mmol/L(P0.05);血清硒水平為51.23μg/L,顯著高于鐵正常組的37.21μg/L(P0.05);而血清鈉、鉀、鎂、鉻、鎳、鎘、錳、銅、鋅離子濃度兩組間均未見明顯差異(P均0.05)。體格檢查結(jié)果分析顯示,鐵過載組體質(zhì)指數(shù)(BMI)為26.31kg/m2,顯著高于鐵正常組25.47 kg/m2(P0.05),收縮壓和舒張壓均顯著高于鐵正常組(P0.05)。肝功能相關(guān)酶類分析顯示,谷丙轉(zhuǎn)氨酶含量為32U/L,顯著高于鐵正常組的22U/L(P0.05);谷草轉(zhuǎn)氨酶含量為22U/L,顯著高于鐵正常組的20U/L(P0.05);谷氨酰轉(zhuǎn)肽酶含量38U/L,顯著高于鐵正常組的19U/L(P0.05)。結(jié)論鐵在機體內(nèi)過量沉積,可導(dǎo)致與鐵代謝相關(guān)指標(biāo)顯著變化,血紅蛋白、紅細(xì)胞數(shù)量、體積及血清中鐵含量顯著增加;初步分析發(fā)現(xiàn)鐵過載人群在BMI偏高的人群(BMI25kg/m2)中多見,肝功能相關(guān)酶的活性顯著增高;然而,機體內(nèi)鐵過量沉積引起的機體血液學(xué)指標(biāo)明顯變化,是否對機體健康帶來潛在危害有待深入研究。
[Abstract]:Objective different levels of iron load can cause hematological changes and bring adverse effects on the body. The purpose of this study was to analyze the changes of clinical hematological characteristics and the effects on some health factors in adult population under the condition of iron overload, and to provide a reference for further study on the effect of iron overload on population health and disease prevention. Methods from January to December 2014, 113 subjects with normal iron status (male: 1515g / L; female: 1515g / L; female: 150 渭 g / L) and 124 iron overload (male: 200 渭 g / L; female: 150 渭 g / L) were randomly selected according to the serum ferritin (serum ferritin) level, aged 4070 years. Fasting venous blood was collected to detect the blood indexes related to iron metabolism, such as hemoglobin, the number of peripheral blood cells, serum iron, transferrin, liver function and mineral level. The differences between the two groups were compared and analyzed. Results the hemoglobin (Hb) average level of iron overload group was 156.16 g / L, which was higher than that of iron normal group 147.76g/L (P0.001). The erythrocyte count in iron overload group was 5.01 脳 10 12 / L, which was significantly higher than that in iron normal group (4.83 脳 10 12 / L, P0.05), and the hematocrit, mean erythrocyte volume and mean erythrocyte hemoglobin content in iron overload group were significantly higher than those in iron normal group (P 0.05). The serum iron content of iron overload group was 22.93 渭 mol / L, which was significantly higher than that of iron normal group (17.83 渭 mol/L), the transferrin saturation of iron overload group was 36%, significantly higher than 28% of iron normal group (P0.05), the unsaturated iron binding ability and transferrin concentration were significantly lower than that of iron normal group (P0.05). There was no statistical difference in total iron binding ability between the two groups (P0.05). Serum protein level analysis showed that the total protein concentration of iron overload group was 76.02 g / L, higher than that of iron normal group 73.64g/L (P0.05), albumin concentration was 44.83 g / L, significantly higher than iron normal group 43.55g/L (P0.05). The serum mineral level in iron overload group was 2.42 mmol / L, which was significantly higher than that in iron normal group (P0.05); the serum selenium level was 51.23 渭 g / L, which was significantly higher than that in iron normal group (37.21 渭 g / L, P0.05); and the serum sodium, potassium, magnesium, chromium, nickel, cadmium, manganese, copper, sodium, potassium, magnesium, chromium, nickel, cadmium, manganese and copper in serum were significantly higher than those in normal iron group (P0.05). There was no significant difference in zinc ion concentration between the two groups (P 0.05). The results of physical examination showed that the body mass index (BMI) of iron overload group was 26.31 kg / m ~ (2), which was significantly higher than that of iron normal group (25.47 kg/m2) (P0.05), and systolic and diastolic blood pressure (DBP) were significantly higher than that of iron normal group (P0.05). Liver function related enzymes analysis showed that the content of glutamate-pyruvic transaminase (32U / L) was significantly higher than that of iron normal group (22U/L) (P0.05); the content of alanine aminotransferase (22U / L) was significantly higher than that of iron normal group (20U/L) (P0.05); the content of glutamyl transpeptidase (38UL) was significantly higher than that of iron normal group (P0.05). Conclusion excessive deposition of iron in the body can result in significant changes in the related indexes of iron metabolism, such as hemoglobin, erythrocyte number, volume and iron content in serum, and the primary analysis shows that iron overload group is more common in the population with higher BMI (BMI25kg/m2). The activity of liver function-related enzymes was significantly increased, however, the changes of hematological parameters caused by excessive iron deposition in the body, whether the potential harm to the health of the body need to be further studied.
