米非司酮頓服后不同時間對早孕蛻膜形態(tài)及類固醇受體表達的影響

【摘要】:目的探討藥物流產中縮短米非司酮用藥時間的可行性。方法將80例因非意愿妊娠自愿要求終止早孕的女性隨機分為4組:對照組(直接負壓吸宮術)、用藥后12 h組、24 h組、48 h組,3個用藥組頓服米非司酮150 mg,于不同時間行負壓吸引術,留取各組蛻膜組織。應用光學顯微鏡、透射電子顯微鏡觀察4組蛻膜組織的形態(tài)學改變,應用免疫組織化學方法和計算機圖像分析系統(tǒng)檢測四組蛻膜組織中類固醇受體的表達。結果 3個用藥組蛻膜細胞變性、壞死,間質水腫、出血,毛細血管擴張充血。對照組蛻膜組織中雌激素受體(ER)和孕激素受體(PR)陽性染色位于大部分蛻膜細胞及少數(shù)腺上皮細胞,用藥后12 h組、24 h組、48 h組蛋白表達均低于對照組,差異有統(tǒng)計學意義(P0.05),而這3組之間無統(tǒng)計學差異(P0.05)。結論米非司酮用藥后12 h蛻膜組織變性壞死,蛋白表達受到抑制,為臨床縮短藥物流產用藥時間提供了一定的理論參數(shù)。
[Abstract]:Objective to study the feasibility of shortening the time of mifepristone in drug abortion. Methods Eighty women who voluntarily requested termination of early pregnancy due to unwanted pregnancy were randomly divided into four groups: control group (direct negative pressure aspiration of uterus), 12 h group, 24 h group, 48 h group, and 3 drug groups taking mifepristone 150 mg, at different time Between negative pressure suction, The decidual tissue was retained in each group. The morphologic changes of decidua tissues were observed by optical microscope and transmission electron microscope. The expression of steroid receptors in decidua tissues was detected by immunohistochemical method and computer image analysis system. Results in the three groups, decidual cell degeneration, necrosis, interstitial edema, hemorrhage, capillary dilatation and hyperemia. The positive staining of estrogen receptor (ERR) and progesterone receptor (PRP) in decidua of control group was found in most decidual cells and a few glandular epithelial cells. The difference was statistically significant (P 0.05), but there was no significant difference among the three groups (P 0.05). Conclusion the degeneration and necrosis of decidua tissue and the inhibition of protein expression at 12 h after mifepristone treatment may provide some theoretical parameters for shortening the time of drug abortion.
