【摘要】：目的本文研究應(yīng)用較廣的10個(gè)診斷相關(guān)組(Diagnosis-Related Groups,DRGs)婦嬰分組,以探討建立適合我國(guó)國(guó)情的婦嬰DRGs體系,以及建模中可汲取的經(jīng)驗(yàn)和需注意的問題。方法采用文獻(xiàn)研究和專家評(píng)議的方法。資料來源于Cochrane Database of Systematic Reviews、MEDLINE、Web of Science、Internet及中國(guó)知網(wǎng)全文數(shù)據(jù)庫。結(jié)果各國(guó)婦嬰DRGs分組邏輯和并發(fā)癥與合并癥再分組方法及分組結(jié)果差異較大,女性生殖系統(tǒng)疾病或紊亂組(MDC13)Nord-DRGs的DRGs分組最少(17個(gè)),GHM的分組最多(94個(gè))。妊娠、分娩和產(chǎn)褥期(MDC14)組DRGs分組最少為MS-DRGs(15個(gè)),最多為CMG+(88個(gè))。新生兒(MDC15)組Adjacent DRGs(ADRGs)和DRGs最少的都為MS-DRGs(6個(gè)ADRGs,7個(gè)DRGs),最多也為CMG+(分別為29組和116組)。結(jié)論我國(guó)的婦嬰DRGs研究與應(yīng)用應(yīng)借鑒國(guó)際經(jīng)驗(yàn),重點(diǎn)關(guān)注婦幼專科特色、分組數(shù)據(jù)標(biāo)準(zhǔn)和質(zhì)量、模型預(yù)測(cè)效率、分組的保健與臨床意義、綜合DRGs研究與管理團(tuán)隊(duì)等。
[Abstract]:Objective to study 10 groups of Diagnosis-Related groups (Diagnosis-Related groups), which are widely used in diagnosis, in order to establish the DRGs system of women and infants suitable for the situation of our country, as well as the experiences and problems to be paid attention to in modeling.Methods the methods of literature study and expert review were used.The data were collected from Cochrane Database of Systematic reviews and MEDLINEN Web of Science Internet and the full text database of China knowledge Network.Results there were significant differences in the grouping logic, complications and complications between women and infants in each country and the results of re-grouping. The DRGs group of MDC13N Nord-DRGs in female reproductive system disease or disorder group was the least (17 cases were the most group) (94 cases).In pregnancy, parturition and puerperal MDC14 group, the DRGs group was MS-DRGs at least (15) and CMG (88) at most.MS-DRGs (6 ADRGs, 7 DRGs) and CMG (29 and 116 groups) were the least in Adjacent DRGs and DRGs in neonatal MDC15 group.Conclusion the research and application of DRGs for women and infants in China should draw lessons from international experience and focus on the characteristics of women and children, the standard and quality of grouping data, the efficiency of model prediction, the health care and clinical significance of grouping, and the comprehensive research and management team of DRGs.
