【摘要】：目的:從多巴胺能神經(jīng)毒性的角度研究職業(yè)性錳接觸對(duì)職業(yè)人群的危害,探討職業(yè)性錳接觸對(duì)工人機(jī)體神經(jīng)損害的影響。方法:對(duì)某鋼鐵集團(tuán)修造公司中錳作業(yè)環(huán)境進(jìn)行監(jiān)測(cè),選擇該鋼鐵企業(yè)中男性接觸錳煙塵危害的電焊工150人作為錳接觸組,同時(shí)將該鋼鐵企業(yè)的能源動(dòng)力廠的男性水處理工162人作為對(duì)照組。收集各組工人一般情況,如工作制度、年齡、職業(yè)工齡史、吸煙等,同時(shí)應(yīng)用精神狀態(tài)量表(MMSE)、畫(huà)鐘測(cè)試(CDT)和語(yǔ)言流暢性方式(VFT)測(cè)驗(yàn)錳接觸組和對(duì)照組工人的神經(jīng)行為認(rèn)知功能、各組工人的個(gè)人癥狀與體征,綜合測(cè)評(píng)其神經(jīng)功能損害程度。依據(jù)國(guó)家職業(yè)衛(wèi)生標(biāo)準(zhǔn),監(jiān)測(cè)作業(yè)環(huán)境空氣中錳及其化合物、NOx、O3的含量;測(cè)定內(nèi)暴露劑量尿錳的濃度用石墨爐原子吸收分光光度法。經(jīng)ELISA試劑盒檢測(cè)對(duì)錳接觸和對(duì)照組工人尿高香草酸(UHVA)表達(dá)水平進(jìn)行檢測(cè),經(jīng)ELISA試劑盒檢測(cè)錳接觸工人和對(duì)照組工人催乳素(PRL)、parkin表達(dá)水平。以尿錳分組,比較不同接觸組中UHVA、PRL、Parkin表達(dá)水平。結(jié)果:在錳接觸組,其作業(yè)場(chǎng)所中的錳及其化合物的濃度(0.120±0.020)、氮氧化物濃度(1.073±0.079)均高于對(duì)照組(0.046±0.023、0.919±0.113),有統(tǒng)計(jì)學(xué)意義(P0.05)。錳接觸組VFT(39.90±2.70)及神經(jīng)行為認(rèn)知功能測(cè)試(72.64±3.11)低于對(duì)照組(41.49±2.08;74.51±2.31),有統(tǒng)計(jì)學(xué)意義(P0.05);且不同接觸組之間比較,高接觸組中VFT(37.45±1.61、69.68±2.16)及神經(jīng)行為認(rèn)知功能測(cè)試均低于中、低接觸組(40.55±2.54、41.13±2.27;74.36±2.51、73.31±2.73),有統(tǒng)計(jì)學(xué)意義(P0.05)。錳接觸組的個(gè)人癥狀與體征調(diào)查中,在乏力、注意力不集中、焦慮、易怒的陽(yáng)性率(11.3%;12.0%;8.7%;6.7%)均高于對(duì)照組(4.9%;5.6%;3.1%;1.9%),均有統(tǒng)計(jì)學(xué)意義(P0.05)。錳接觸組(6.78±1.395)與對(duì)照組UHVA水平(4.97±1.242)比較,差異有統(tǒng)計(jì)學(xué)意義(P0.05),在錳接觸組中,不同接觸組間UHVA水平比較,中接觸組(7.10±1.493)與低工齡(6.37±1.128)和高接觸組(6.54±1.279)比較,差異均有統(tǒng)計(jì)學(xué)意義(P0.01)。錳接觸組(12.63±3.108)與對(duì)照組(11.10±2.819)的血清PRL表達(dá)水平比較,差異有統(tǒng)計(jì)學(xué)意義(P0.05),錳接觸組中不同接觸組之間血清PRL表達(dá)水平經(jīng)倆倆比較,中接觸組(14.02±2.859)與低工齡(11.56±2.865)、中接觸組與高接觸組(10.90±2.453)比較,差異均有統(tǒng)計(jì)學(xué)意義(P0.01)。錳接觸組在parkin表達(dá)水平(0.559±0.219)上低于對(duì)照組(0.639±0.187),有統(tǒng)計(jì)學(xué)意義(P0.05),錳接觸組中不同接觸組之間parkin表達(dá)水平經(jīng)倆倆比較,高接觸組parkin蛋白表達(dá)水平(0.655±0.220)明顯低于中、低接觸組(0.574±0.212、0.436±0.176),有統(tǒng)計(jì)學(xué)意義(P0.05)。相關(guān)性分析顯示,其中神經(jīng)行為認(rèn)知功能測(cè)試與UHVA、血清PRL表達(dá)水平呈負(fù)相關(guān)關(guān)系((rHVA=-0.577,P0.01;(rPRL=-0.394,P0.01),神經(jīng)行為認(rèn)知功能測(cè)試與Parkin表達(dá)水平呈現(xiàn)正相關(guān)關(guān)系(rparkin=0.675,P0.01)。結(jié)論:Ⅰ.職業(yè)性錳接觸引起了一定的神經(jīng)行為認(rèn)知功能改變。Ⅱ.錳接觸改變了多巴胺能神經(jīng)遞質(zhì)表達(dá)水平,且隨錳接觸的時(shí)間,影響其神經(jīng)系統(tǒng)行為認(rèn)知功能以及多巴胺能神經(jīng)遞質(zhì)表達(dá)的程度。Ⅲ.parkin蛋白表達(dá)水平可能作為早期錳中毒的神經(jīng)毒性指標(biāo)。

The ELISA kit to detect the contact and control workers urinary homovanillic acid (UHVA) on manganese levels were detected by ELISA kit, detection and control of manganese exposed workers group of workers (PRL), prolactin. The expression level of parkin in urine manganese group, UHVA, compare the different contact group PRL, the expression level of Parkin.
