職業(yè)緊張與尿液中部分激素代謝物水平的關(guān)系

【摘要】:目的 職業(yè)緊張越來越受到人們的關(guān)注,目前國內(nèi)外評價職業(yè)緊張的主要方法是使用心理學量表和問卷,而對職業(yè)緊張的客觀評價指標研究較少。由于心理學工具的評價結(jié)果受主觀因素影響較大,因此開展一些職業(yè)緊張的客觀評價指標的研究就顯得尤為重要。 近年來,國內(nèi)外開展了一些職業(yè)緊張客觀評價指標的研究,但實驗室研究居多,指標介質(zhì)多為唾液和血液,且得出確定性結(jié)論的較少,而以尿液為介質(zhì)的現(xiàn)場研究開展不多。為此,本次研究增大樣本量,開展以隨機尿液為指標介質(zhì)的現(xiàn)場人群研究,旨在了解真實工作條件下,職業(yè)緊張及其相關(guān)因素與尿液中17-羥皮質(zhì)類固醇(17-hydroxy-cortico-steroid,17-OHCS)、17-酮皮質(zhì)類固醇(17-keto-steroid,17-KS)、香草扁桃酸(vanillylmandelic acid,VMA)及高香草酸(homovanillic acid,HVA)水平之間的關(guān)系,探討尿液中17-OHCS、17-KS、VMA及HVA作為職業(yè)緊張評價指標的可能性,為職業(yè)緊張的識別與評價提供新的科學依據(jù)。 方法 以整群抽樣的方法,抽取鄭州市公安局某派出所的225名在職民警作為研究對象。使用職業(yè)緊張量表測試其職業(yè)緊張因素、緊張反應、個性特征和緊張緩解因素等。采用酶聯(lián)免疫吸附試驗和高效液相色譜-熒光檢測法對尿液中17-羥皮質(zhì)類固醇(17-OHCS)、17-酮皮質(zhì)類固醇(17-KS)、香草扁桃酸(VMA)及高香草酸(HVA)水平進行檢測。 應用SPSS12.0軟件包對人口統(tǒng)計學特征和職業(yè)緊張相關(guān)因素不同組間的尿液激素代謝物水平差異分別進行t檢驗或單因素方差分析,以人口統(tǒng)計學特征和職業(yè)緊張相關(guān)因素為自變量,尿液激素代謝物水平為因變量,進行多因素非條件Logistic回歸等統(tǒng)計學分析。檢驗水準為α=0.05(雙側(cè))。 結(jié)果 1人口學特征不同組間激素代謝物水平差異的t檢驗結(jié)果顯示,男性、女性的尿液17-OHCS、17-KS、VMA和HVA水平間的差異有統(tǒng)計學意義(均P0.05);飲酒的不同組間比較,17-OHCS、17-KS的水平差異具有統(tǒng)計學意義(均P0.05);不同警種間激素代謝物水平差異比較顯示,交警與戶籍警、刑警的17-KS水平的差異有統(tǒng)計學意義(均P0.05)。 2緊張相關(guān)因素不同評分組間的尿液激素代謝物水平比較結(jié)果顯示,同事支持、負性情緒的高、低水平組間的17-OHCS水平差異具有統(tǒng)計學意義(t=2.330,P=0.021;t=-2.017,P=0.034);工作保障的高、低水平組間的17-KS水平差異具有統(tǒng)計學意義(t=3.248,P=0.001);同事支持、非工作壓力的高、低水平組間的VMA水平差異具有統(tǒng)計學意義(t=3.174,P=0.002,t=-2.319,P=0.022);應付策略的高、低水平組間的HVA水平差異具有統(tǒng)計學意義(t=1.985,P=0.046)。 3以尿液激素代謝物水平為因變量進行多因素非條件Logistic回歸分析顯示,飲酒和工作控制、尊重均與尿液17-OHCS水平呈負向關(guān)系(OR=0.385,P=0.045;OR=0.353,P=0.017;OR=0.352,P=0.045);戶籍警、刑警、內(nèi)在投入、自尊感和每日緊張感與尿液17-OHCS水平呈正向關(guān)系(OR=3.409,P=0.018;OR=3.600,P=0.020;OR=3.391,P=0.013;OR=2.439,P=0.041;OR=2.584,P=0.048)。刑警與尿17-KS水平呈正向關(guān)系(OR=2.949,P=0.037);工作控制和工作保障均與尿17-KS水平呈負向關(guān)系(OR=0.387,P=0.026;OR=0.356,P=0.014)。負性情緒與尿液VMA水平呈正向關(guān)系(OR=2.643,P=0.022)。所有人口統(tǒng)計學特征和職業(yè)緊張相關(guān)因素均未進入以HVA水平為因變量的方程。 結(jié)論 工作控制水平低、尊重水平低、內(nèi)在投入水平高、自尊感水平高和每日緊張感水平高可導致尿液中17-OHCS水平的升高,工作控制水平低和工作保障水平低可導致尿液中17-KS水平的升高,負性情緒水平高可導致尿液中VMA水平的升高;尿液中17-OHCS、17-KS和VMA水平有作為職業(yè)緊張客觀評價指標的可能性,但需做進一步研究。
[Abstract]:Purpose Occupational stress is getting more and more attention . At present , the main method of evaluating occupational stress at home and abroad is to use psychological scale and questionnaire , and to study the objective evaluation index of occupational stress . As the result of evaluation of psychology tool is influenced by subjective factors , the research on objective evaluation index of some occupational stress is very important . In recent years , there have been studies on objective evaluation indexes of occupational stress at home and abroad , but there are not many laboratory studies , the index media is mostly saliva and blood , and there are few field studies with urine as medium . In order to understand the relationship between the levels of 17 - hydroxy - cortico - steroid ( 17 - OHCS ) , 17 - keto - steroid ( 17 - KS ) , vanillyllic acid ( VMA ) and homovanillic acid ( HVA ) in urine , the paper discusses the possibility of 17 - OHCS , 17 - KS , VMA and HVA in urine as the index of occupational stress assessment , and provides a new scientific basis for the identification and evaluation of occupational stress . method 225 active civilian police officers from a police station of Zhengzhou Public Security Bureau were taken as the subjects . The occupational stress factors , stress responses , personality traits and strain relief factors were tested by using the occupational stress scale . The levels of 17 - hydroxycorticosteroid ( 17 - OHCS ) , 17 - keto corticosteroids ( 17 - KS ) , vanilla - tonine acid ( VMA ) and high - vanillic acid ( HVA ) were tested by enzyme - linked immunosorbent assay and high performance liquid chromatography - fluorescence detection . SPSS 12.0 software was applied to analyze the difference of urine hormone metabolites level between different groups of demographic characteristics and occupational stress . The statistical analysis of demographic characteristics and occupational stress - related factors was carried out . The statistical analysis of multivariate non - conditional logistic regression was conducted . The test level was 偽 = 0.05 ( bilateral ) .
